MEMORANDUM OF CONSIDERATION OF APPLICATION BEFORE THE AFBCMR 
SUBJECT : 

Having carefully reyiewed this application, we agree with 

the recommendation of the Air Force office of primary 
responsibility and adopt the rationale expressed as the basis for 
our decision that the applicant has been the victim of either an 
error or an injustice.  Therefore, under the authority delegated 
in AFI 36-2603, the applicant's records will be corrected as set 
forth in the accompanying Memorandum for the Chief of Staff 
signed by the Executive Director of the Board or his designee.

SUBJECT: Application for Correction of Military Records (DD Form 149) 

1.  Applicant has submitted a request for correction of military records in regards to his self- 
initiated elimination (SIE) from T-4 1 training while attending the United States Air Force 
Academy (USAFA).

2.  Applicant believes a situation beyond his control lead to his self-initiated elimination from the 
T-4 1 pilot indoctrination program while attending USAFA. 

3.  Applicant was enrolled in the T-41 class 95-05 at USAFA during his third summer training 
period (summer 94) prior to his first class year (senior).  Applicant flew his first sortie on 19 Jul 
94 followed by four sorties on:  20 Jul, 21 Jul, 22 Jul and 26 Jwl.  This is reflected by the ATC 
Form 860 in the applicant’s attached T-41 records.  The applicant’s progress was above average 
to that point.  The first documented indication of the applicant’s desire to a SIE from T-41 
training came 27 Jul94, as evidenced by the ATC 803A in the applicant’s attached T-41 records. 
In the 803A dated 27 Jul94, the applicant’s flight commander wrote, “(Applicant) has stated a 
desire to SIE.  (Applicant) is dealing with many issues right now with the death of his friend, 
issues with his family, and his own future goals in life.  We discussed his situation in-depth and 
he seems to be making a logical, well thought-out decision. Performance to this point has been 
strong and attitude is excellent.”  On 29 Jul94, the applicant was counseled by the 557 FTS 
squadron commander and operations officer.  An ATC Form 126A dated 29 Jul94 was signed by 
the applicant, the 557 FTS squadron commander, and operations officer.  The applicant also 
signed on 29 Jul94 a letter of declination of Undergraduate FIying Training (UFT), where it was 
clearly spelled out that if the applicant was an SIE he would be ineligible to apply for UFT in the 
fbture.  The applicant’s T-41 records were faxed to the 12th Operations Group at Randolph AFB, 
TX for review, and the operations group commander, the elimi

4.  The summer of 1994 was the last time screening was conducted using the T-41.  Fall 94 was 
used as the transition period to a new aircraft, the T-3A Firefly, and student screening resumed 
the spring of 1995 using the T-3A for screening cadets.  The applicant signed a letter of 
declination of Undergraduate Flying Training on 29 Jul94.  Air Force Instruction 36-2205, 
figure 1.1 clearly states that those eliminated for self-initiated elimination (SIE) are ineligible for 
further flying training.  .The applicant’s SIE records were typical for that time frame and no major 
discrepancies were noted. 

5.  Recommendation:  Remove the declination of U 
29 Jul94 and ATC Form 126A dated 29 Jul94 from 
training record.  The circumstances behind 
elimination from the T-41 program was driven by the  eath of his friend in an automobile 
accident.  I feel there is sufficient justification to remove these documents from- 
record and that this course of action is in the best interest of the Air Force 

case indicate his self-initiated v
