After careful consideration of applicant's request and the
available evidence of record, we find insufficient evidence of
error or injustice to warrant corrective action. The facts and
opinions stated in the advisory opinions appear to be based on
the evidence of record and have not been rebutted by applicant.
Absent persuasive evidence applicant was denied rights to which
entitled, appropriate regulations were not followed, or
appropriate standards were not applied, we find no basis to
disturb the existing record.
Accordingly, applicant's request is denied.
The Board staff is directed to inform applicant of this decision.
Applicant should also be informed that this decision is final and
will only be reconsidered upon the presentation of new relevant
evidence which was not reasonably available at the time the
application was filed.

direct support of the operations in Vietnam. To be eligible, a service member must be (1)
attached to or regularly serving for one or more days with an organization participating in or
directly supporting ground (military) operations; (2) attached to or regularly serving for one or
more days aboard a naval vessel directly supporting military operations; (3) actually participate
as a crew member in one or more aerial flights directly supporting military operations; or (4)
serve on temporary duty (TDY) for 30 consecutive or 60 nonconsecutive days.
4. DISCUSSION. Although the applicant served a tour in Thailand, it was over a year after the
closeout date for award of the Vietnam Service Medal. Applicant did not provide orders assign-
ing him as a “regularly assigned crew member,” TDY orders showing he was assigned to Viet-
nam in direct support of the operations in Vietnam, or Travel Voucher(s) showing the inclusive
dates he was in Vietnam. The only documentation provided was a copy of a medal report show-
ing he reported to a dispensary in Vietnam on 18 Jan 68. On 5 Dec 67 and 3 May 68, entries

nam Service Medal.
5. RECOMMENDATION.
We recommend disapproval of the applicant’s request to correct his DD Form 214 to “show
Vietnam War service dates’’ and Vietnam Service Medal.

1. REQUESTED ACTION: Applicant requests the DD Form 214 be amended to
reflect “all schools ... listed”.
2. BASIS FOR REQUEST: Applicant identified several course titles, but
initially provided no source documents to verify course completion.
3. FACTS: In order to honor the applicant’s request, supporting documentation
such as copies of certificates of completion for training or official transcripts for
academic courses must be provided to verify training. There are no documents in the
applicant’s master personnel records to support the request nor did the applicant provide
appropriate documentation. A letter was sent (dated: 15 Jul98) to the individual
specifjling the need for specific source documents to verify course completion. Applicant
was advised to forward documents within 45 days or request an extension if necessary.
As of this date, no documents have been forwarded.
5. RECOMMENDATION: Recommend denial of applicant’s request since no
support documentation is available.
