AIR FORCE BOARD FOR CORRCCTION OF MILITARY RECORDS

RECORD OF PXGZEEDINGS

IN THE MATTER OF:

DSCKET NUMBER: 98 00698

COUNSEL: None

EEARING DESIRED: NO

Applicant requests his DD Form 214 (Certificate o f Release o r
Discharge From Active Duty) be corrected in Block 7A (Place of
Entry Into Active Duty) to read Jackson, Missi.ssippi, and the
Fourth Oak Leaf Cluster ( 4 0 L C : l be added to the Meritorious
Service Medal (MSM) in Block 13 {Decorations, Medals, Badges,
Citations and Campaign Ribbons Awarded or Authorized)
Applicant's submission is at Exk:bit A.

The appropriate Air Force office evaluated applicant's request
and provided an advisory opinicn tc the Board recommending the
application be denied (Exhibit Z .
The advisory opinion was
forwarded to the applicant for- review and response (Exhibit Dj.
As of this date, no response has Deex received by tliis office.

We note that applicant's request pertaining to Block 7A of his DD
Form 214 has been corrected administratively; therefore, the only
issue before this Board Is his r - e q i s s t that the 40LC be added to
the MSM in Block 13. It appears that the applicant was requested
by AFPC/DPPPRA to provide copies of the orders fox- award of the
third anc fourth OLCs to ensure his records were ccmpiete and up
to date; however, he did not r-espond. In view of the above and
after noting that without a copy o f -,he orders awarding the 40LC
to the MSM, applicant's eligibility for, or- entitlement to, this
decoration cannot be ver-ified. Therefore, the facts, and o p i n i o n s
stated in the advisory opiniori appear- to be based orL the evidence
of record and have not been rebutted by applicant.
Absent
was denied rights to which
persuasive evidence applicant
ions were not followed, or
entitled, appropriate regulat
applied, we find no basis to
appropriate standards were not
M S M tc his DD Form 214.
recommend adding the 4 0 L C to the

Accordingly, applicant's request

The Board staff is directed to inform applicant of this decision.
