Applicant requests that his discharge order, issued by the Air
Force Reserve, be amended to correct his service number and to
show he was promoted to sergeant. It was noted that applicant's
service number will be administratively corrected by the
respective Air Force office; therefore, no action is required by
this Board. Applicant's submission is at Exhibit A.
The appropriate Air Force office evaluated applicant's request
and provided an advisory opinion to the Board recommending the
application be denied (Exhibit C).
The advisory opinion was
forwarded to the applicant for review and response (Exhibit D).
As of this date, no response has been received by this office.
After careful consideration of applicant's request and the
available evidence of record, we find insufficient evidence of
error or injustice to warrant corrective action concerning his
grade at the time of discharge. The facts and opinions stated in
the advisory opinion appear to be based on the evidence of record
and have not been rebutted by applicant.
Absent persuasive
evidence applicant was denied rights to which entitled,
appropriate regulations were not followed, or appropriate
standards were not applied, we find no basis to disturb the
existing record.
Accordingly, applicant's request is denied.
The Board staff is directed to inform applicant of this decision.
Applicant should also be informed that this decision is final and
will only be reconsidered upon the presentation of new relevant
evidence which was not reasonably available at the time the
application was filed.

SUBJECT: Application for Correction of Military Records,
1 . The requested correction cannot be accomplished administratively at this headquarters.
2. The applicant requests his discharge order be amended to correct his service number and to show he
was promoted to Sergeant.
3. The following is an analysis of the case:
a. The applicant was discharged from the Air Force Reserve on 4 Dec 50 for physical disqualification,
with no active service as a reservist per AF Reserve Order 261, dated 6 Dec 50. This order lists his
service number as AF65000045 and a rank of Corporal. He states that the quartermaster gave him his
insignia and he had been upgraded to sergeant.
b. The applicant’s service number was incorrect on his discharge order and should have read AF
56000045. However, there are no documents in the applicant’s records indicating he was promoted to
Sergeant.

4. Recommend the applicant’s request to have the grade on his discharge certificate amended to read
Sergeant be denied. There are no documents in his records indicating he was promoted to sergeant. The
applicant’s request to have his service number corrected on his discharge order can be accomplished.
However, we are not authorized to make the correction since the order was originally published by
HQ 4th AF. We will forward the applicant’s request and supporting documents to 4th AF to have the
order amended,

On 2 Feb 91, he enlisted in the Washington Air National Guard (ANG); he transferred to the Air Force Reserve on 10 Dec 92. On 19 Feb 95, he was discharged from the Air Force Reserve with a General discharge by reason of Defective Enlistment – Fraudulent Entry. On 5 Jul 94, HQ AFRES/CV approved the findings and recommendations and the case file was forwarded to the Air Force Personnel Board (AFPB) for a determination as to whether the applicant should receive Lengthy Service Probation (LSP).
