         910905:  Mental status evaluation and physical examination cleared applicant               for separation.
         911009:  Unit commander notified the applicant of initiation of separation                 action under the provisions of Chapter 14, AR 635-200, for                         misconduct - pattern of misconduct. He was advised of his rights.
         911009:  Applicant acknowledged notification.
         911105:  Applicant consulted legal counsel, and did not submit a statement in              his own behalf.
         911106:  Unit commander recommended separation with a general under honorable              conditions discharge.
