Note: Applicant’s mental status evaluation and physical examination

930503: Unit commander notified the applicant of initiation of
separation action under the provisions of Chapter 14,
AR 635-200, for pattern of misconduct. She was advised
of her rights.
 930510: Applicant acknowledged notification, consulted legal counsel,
waived a hearing by a board of officers, and submitted a
statement in her own behalf.
930510: Applicant again consulted with legal counsel, voluntarily waived
consideration of her case by an administrative separation board
contingent upon her receiving a characterization of service no
less favorable than an honorable discharge.
930511: Unit commander recommended separation with a general discharge.
930511: Unit commander recommended disapproval of the request for
a conditional waiver.
930511: Intermediate commander concurred with the unit commander’s
recommendation.
930527: Senior intermediate commander recommended disapproval, and requested
