THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show:

	a.  his last name as J----- instead of F----; and

	b.  he completed the Ranger Course and the Air Assault Course. 

2.  The applicant states that he completed these courses and they should be added to his DD Form 214.  Changing his last name on his records would help as he is continuing his career in the National Guard under his new last name.

3.  The applicant provides:

* Ranger Course Certificate, dated 28 August 2009
* Ranger Tab orders, dated 24 August 2009
* Air Assault Course Certificate, dated 11 December 2009
* his DD Form 214 for the period ending 11 December 2011
* Davis County Utah court document showing he changed his last name effective 11 June 2012

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army (RA) on 15 August 2006 using the last name F----.  He held military occupational specialty 11B (Infantryman).


2.  His Enlisted Record Brief (ERB) shows in Section VI (Military Education), in part, he completed the Ranger Course and the Air Assault Course in 2009.

3.  He was released from active duty (REFRAD) effective 11 December 2011 as a sergeant/E-5.  He had completed 5 years, 3 months, and 27 days of active service.  There is no available evidence showing he used any last name other than F---- during his RA service.

4.  The Army Training Requirements and Resources System shows the U.S. Army's Ranger Course consists of 20 days of training conducted by the 75th Ranger Regiment at Fort Benning, GA.  The purpose of the course is to prepare Army officers and enlisted Soldiers in combat arms-related functional skills.  The course teaches the fundamentals of small unit leadership and patrolling. 

5.  The Fort Benning, GA, website shows the Air Assault Course is 12 days in duration and prepares Soldiers for air mobile operations.  During the course students train and are evaluated on combat assault, sling loads, rappelling, physical fitness, and various other critical skills.

6.  Army Regulation 635-5 (Separation Documents), in effect at the time, prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  This regulation has historically stated that item 1 will contain the name taken from the Soldier's personnel record.  It further states for item 14: From ERB, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. Entering of in-service training courses is accomplished to assist Soldiers after separation in job placement and counseling; therefore, training courses for combat skills will not be listed

DISCUSSION AND CONCLUSIONS:

1.  The evidence shows he completed the Ranger Course and the Air Assault Course in 2009.  However the regulatory guidance specify that training courses for combat skills will not be listed on the DD Form 214.  Therefore, there is no basis for adding these courses to his DD Form 214.

2.  He enlisted and served under the last name F----.  He provides a court document showing he changed his last name to J----- effective 11 June 2012, which is after his REFRAD.  Therefore, there is also no basis for granting this portion of his request.
