THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show the rank of captain (CPT), the pay grade of O-3, and the date of rank (DOR) as 20 July 2014.

2.  The applicant states his promotion orders were published after his DD Form 214 was issued, but before he was released from active duty. 

3.  The applicant provides his DD Form 214 and his promotion orders.  

CONSIDERATION OF EVIDENCE:

1.  The applicant was appointed as a U.S. Army Reserve second lieutenant (2LT) in the Medical Corps and he executed an oath of office on 14 May 2010.  On     23 January 2012, he entered active duty as a first lieutenant (1LT).   

2.  He was honorably released from active duty on 30 September 2014.  The      DD Form 214 issued to him at the time shows the following entries:

* item 4a (Grade, Rate, or Rank) "1LT" 
* item 4b (Pay Grade) "O02"
* item 12i (Effective Date of Pay Grade) "21 November 2011"

3.  Orders B-08-405510, issued by U.S. Army Human Resources Command, Fort Knox, KY, dated 25 August 2014, show he was promoted to CPT/O-3 effective  20 July 2014.  
4.  Army Regulation 635-5 (Separation Documents) is a synopsis of the Soldier's most recent period of continuous active service.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that his rank and pay grade are incorrect on his 
DD Form 214 was carefully considered and determined to have merit.

2.  Orders show that he was promoted to the rank of CPT/pay grade O-3 effective 20 July 2014.  Therefore, it would be appropriate in this case to correct his DD Form 214 by amending item 4a to show the entry "CPT," item 4b to show the entry "O03," and item 12i to show the entry "20 July 2014."

As a result, the Board recommends that the DD Form 214 of the individual concerned be corrected by amending item 4a to show the entry "CPT," item 4b to show the entry "O03," and item 12i to show the entry 
"20 July 2014."
