THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests items 4a (Grade, Rate or Rank) and 4b (Pay Grade) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his rank as major.
2. The applicant states:

* his DD Form 214 should reflect the rank of major

CONSIDERATION OF EVIDENCE:
1. Having prior commissioned service in the U.S. Army Reserve, he was ordered to active duty in the rank of captain on 14 June 2005 to fulfill his Active Army requirement. On 1 July 2010, he was honorably discharged for miscellaneous/general reasons.
2. Item 4a of his DD Form 214 shows the entry "CPT" [captain]. Item 4b shows the entry "O03." Item 12i (Effective Date of Pay Grade) shows the entry "2005 06 14" [14 June 2005].
3. There are no promotion orders in the available records which show he was promoted to major.
4. He provided FY 2010 Major, Medical Corps, Selection Board Results, dated
22 June 2010.
DISCUSSION AND CONCLUSIONS:
1. He contends he was promoted to major on 22 June 2010. However, the board results he provided only indicate he was selected for promotion to major, not that he was promoted.
2. In the absence of promotion orders to major, there is insufficient evidence on which to amend items 4a and 4b of his DD Form 214.
