THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Good Conduct Medal (AGCM) (5th Award).
2. The applicant states he received the award after his separation from active duty.
3. The applicant provides a DD Form 214 and orders for the AGCM (5th Award).
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 15 June 1999.
2. Permanent Order Number 162-002, issued by Headquarters, 72nd Brigade Support Battalion, Fort Bliss, TX on 6 June 2014, awarded him the AGCM (5th Award) for the period 13 June 2011 to 14 June 2014.
3. He was retired by reason of disability on 6 July 2014 and placed on the Temporary Disability Retired List on 7 July 2014. His DD Form 214 shows the AGCM (4th Award) among other awards. It does not show the AGCM (5th Award).
4. Army Regulation 635-8 (Separation Processing and Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge.
DISCUSSION AND CONCLUSIONS:
Permanent orders confirm the applicant was awarded the AGCM (5th Award); however, this award was not recorded on his DD Form 214. Therefore, it would be appropriate to correct his DD form 214 by adding the AGCM (5th Award).

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from his DD Form 214 the Army Good Conduct Medal (4th Award) and adding to his DD Form 214 the Army Good Conduct Medal (5th Award).
