THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 23 July 2013 be corrected to show one bronze service star for his Afghanistan Campaign Medal and two bronze service stars for his Iraq Campaign Medal.  He requests the Louisiana Emergency Service Medal be added to his DD Form 214.

2.  The applicant states he was deployed to Afghanistan once and to Iraq twice.  The Louisiana Emergency Service Medal is not on his DD Form 214.

3.  The applicant provides his:

* Enlisted Record Brief (ERB), dated 17 May 2011
* DD Form 214
* a memorandum, dated 8 March 2006, from the Joint Force Headquarters - Louisiana, Office of the Adjutant General 

CONSIDERATION OF EVIDENCE:

1.  On 25 November 1992, he enlisted in the Regular Army and served continuously until his retirement.

2.  His ERB shows he was deployed to:

* Afghanistan from 15 July 2002 to 14 January 2003 and participated in one campaign
* Iraq from 15 August 2003 to 15 April 2004 and participated in one campaign
* Iraq from 10 August 2006 to 19 October 2007 and participated in two campaigns

3.  A memorandum, dated 8 March 2006, from the Joint Force Headquarters - Louisiana, Office of the Adjutant General awarded him the Louisiana Emergency Service Medal for participation in Hurricane Katrina/Rita relief efforts.

	a.  The medal is normally awarded to members of the National Guard who, upon being called to service for a tour of state emergency duty as declared by the Governor of Louisiana, served honorably until relieved from such duty.

	b.  An exception was made by the Adjutant General to award this medal to all active and reserve forces who participated in this relief effort.

4.  On 23 July 2013, he was retired and placed on the Retired List by reason of disability, permanent (enhanced).  His DD Form 214 shows he is authorized the National Defense Service Medal, the Afghanistan Campaign Medal with two bronze service stars, and the Iraq Campaign Medal.  The Louisiana Emergency Service Medal is not shown.

5.  Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. 

	a.  The National Defense Service Medal is awarded for honorable active service for any period from 2 August 1990 to 30 November 1995 and from 11 September 2001 to a date to be determined.  Second and subsequent awards of the National Defense Service Medal are denoted by a bronze service star affixed to the National Defense Service Medal.

	b.  The Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF).  The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area.  The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Approved designated Afghanistan campaigns are:

* Liberation of Afghanistan (11 September 2001-30 November 2001)
* Consolidation I (1 December 2001-30 September 2006)
* Consolidation II (1 October 2006-30 November 2009)
* Consolidation III (1 December 2009 – 30 June 2011)
* Transition I (1 July 2011-date to be determined)

	c.  The Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom.  The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles.  The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 through 31 December 2011.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Approved campaigns are:

* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 August 2010)
* New Dawn (1 September 2010-31 December 2011)

	d.  The Louisiana Emergency Service Medal is not listed in this regulation.

DISCUSSION AND CONCLUSIONS:

1.  He is authorized two awards of the National Defense Service Medal based on his service from 25 November 1992 to 23 July 2013.  Therefore, his DD Form 214 should be corrected to show the National Defense Service Medal (2nd Award).

2.  He participated in one campaign in Afghanistan.  Therefore, his DD Form 214 should be corrected to show the Afghanistan Campaign Medal with one bronze service star.

3.  He was deployed to Iraq twice.  During his first deployment he participated in one campaign.  During his second deployment he participated in two campaigns. Therefore, his DD Form 214 should be corrected to show the Iraq Campaign Medal with three bronze service stars. 

4.  State awards are not included on DD Forms 214.  Therefore, although the Adjutant General of the State of Louisiana awarded him the Louisiana Emergency Service Medal it is not authorized to be entered on the DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 with a separation date of 23 July 2013 as follows:

	a.  delete the National Defense Service Medal and add the National Defense Service Medal (2nd award);

	b.  delete the Afghanistan Campaign Medal with two campaign stars and add the Afghanistan Campaign Medal with one bronze service star; and

	c.  delete the Iraq Campaign Medal and add the Iraq Campaign Medal with three bronze service stars.

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to the Louisiana Emergency Service Medal.
