THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests removal of an Army Good Conduct Medal (AGCM) order from her official records.
2. The applicant states, in effect, that her Army Military Human Resource Record contains two orders awarding her the first award of the AGCM and both orders contain different dates. Accordingly, one order should be removed.
3. The applicant provides copies of both AGCM orders.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 9 September 1993. She completed her training as a chaplainÂ’s assistant and remained on active duty through a series of continuous reenlistments.
2. On 10 December 1996, Permanent Orders Number 345-001 issued by the 90th Personnel Service Battalion awarded the applicant the AGCM (1st Award) for the period 9 September 1993 to 8 September 1996.
3. On 19 May 1997, Permanent Orders Number 139-062 issued by the 90th Personnel Service Battalion awarded the applicant the AGCM (1st Award) for the period 10 September 1993 to 9 September 1996.
4. On 24 May 2006, the applicant was honorably discharged in the pay grade of E-6 in order to accept a commission in the Army.
5. She was commissioned as a second lieutenant on 25 May 2006 with a concurrent call to active duty.
6. A review of her official records failed to show any revocation orders for either of her AGCM orders.
DISCUSSION AND CONCLUSIONS:
1. The first AGCM order (Permanent Order 345-001) contains the correct dates for her AGCM and should be retained in her AMHRR.
2. However, the second order, Permanent Order 139-062 issued on 19 May 1997 contains incorrect dates and should be revoked and removed from her AMHRR.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by revoking Permanent Order 139-062 issued on 19 May 1997, removing it from her AMHRR, and ensuring the revocation orders are not filed in her AMHRR.
