THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he served in Afghanistan from 22 June 2011 to 22 June 2012 vice Iraq.
2. The applicant states he was deployed to Afghanistan from 22 June 2011 to 22 June 2012; however, his DD Form 214 shows this period of service as Iraq.
3. The applicant provides his DD Form 214 and orders.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 11 March 1999. He served through several reenlistments and he was promoted to the rank of sergeant on 1 November 2004.
2. He provides Orders SW-173-0138, dated 22 June 2011, issued by Headquarters (HQ), U.S. Army Garrison, Schweinfurt, Germany, deploying him to Afghanistan in support of Operation Enduring Freedom (OEF) as a member of his unit, Battery B, 77th Field Artillery Battalion, 172th Infantry Brigade, Germany, with a will proceed date of 13 July 2011.
3. His record contains Permanent Orders (PO) 022-026, dated 16 June 2012, issued by HQ, 172nd Infantry Brigade, Afghanistan, awarding him the Army Commendation Medal for exceptionally meritorious service with Task Force Falcon in support of OEF, Afghanistan, from 13 July 2011 to 16 June 2012.
4. His record contains a DA Form 2166-8 (Noncommissioned Officer Evaluation Report (NCOER)) covering the rating period 1 February 2011 through 31 January 2012, wherein in Part III (Duty Description) it states he served as the Ammunition Team Chief in direct support of the 172nd Infantry Brigade while deployed to Afghanistan.
5. He was honorably discharged on 3 August 2013.
6. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the DD Form 214 he was issued does not show award of the Afghanistan Campaign Medal. Item 18 (Remarks) of this form, in part, shows he served in Iraq from 22 June 2011 to 22 June 2012.
7. On 19 February 2014, the Defense Finance and Accounting Service (DFAS) verified the applicant received hostile fire (HF) and imminent danger pay (IDP) for service in Kuwait from 15 July 2011 to 16 June 2012.
8. Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards. It states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF. The period of eligibility is on or after 11 September 2001 to a date to be determined. A bronze service star is authorized for wear with this medal for participation in each credited campaign. A review of his records shows during his service in Afghanistan he participated in one campaign.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record confirms the applicant served in Kuwait/Afghanistan from 15 July 2011 to 16 June 2012 as evidenced by DFAS stating that was the period he received HF/IDP. It appears an error occurred during the preparation of his DD Form 214 when the dates of this service were entered incorrectly and the place he served was entered as Iraq instead of Afghanistan.
2. He served in Afghanistan during a qualifying period for award of the Afghanistan Campaign Medal and he participated in one campaign during this service. Therefore, he is entitled to award of the Afghanistan Campaign Medal with one bronze service star and correction of his DD Form 214 to show this medal.
3. With respect to showing his service in Afghanistan from 22 June 2011 to 22 June 2012, the evidence confirms he served in Kuwait/Afghanistan from 15 July 2011 to 16 June 2012. Therefore, he is not entitled to full relief.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 as follows:
* adding to item 13 the Afghanistan Campaign Medal with one bronze service star
* deleting from item 18 the entry "SERVICE IN IRAQ 20110622-20120622" and replacing it with the entry "SERVICE IN KUWAIT/AFGHANISTAN FROM 20110715-20120616"
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to service in Afghanistan from 22 June 2011 to 22 June 2012.
