THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Purple Heart and the 8th award of the Army Good Conduct Medal.

2.  The applicant states he was awarded the Purple Heart for wounds received in Iraq in 2009 but the award was not added to his DD Form 214.

3.  The applicant provides a DA Form 4187 (Personnel Action), certificate and Permanent Order (PO) 174-017G awarding him the Purple Heart, and PO 016-005 awarding him the Army Good Conduct Medal (8th Award).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 22 February 1989.  He served through multiple extensions or reenlistments in a variety of stateside or overseas assignments and he attained the rank/grade of first sergeant (1SG)/E-8.

2.  He retired on 28 February 2013 and he was placed on the Retired List in his retired rank/grade of 1SG/E-8.  He completed 24 years and 9 days of active service.  His DD Form 214 lists multiple awards, including the Army Good Conduct Medal (7th Award) but not the Purple Heart. 

3.  PO 174-017G, issued by Headquarters, 25th Infantry Division, Contingency Operating Base Speicher, Iraq dated 23 June 2009 awarded him the Purple Heart for wounds received as a result of enemy or hostile action on 6 June 2009. 
4.  PO 016-005, issued by Headquarters and Headquarters Company, 2nd Heavy Brigade Combat Team, 1st Cavalry Division, Fort Hood, TX, dated 
16 January 2013 awarded him the Army Good Conduct Medal (8th Award) for service from 21 February 2010 to 20 February 2013. 

DISCUSSION AND CONCLUSIONS:

1.  PO awarded the applicant the Purple Heart which is not shown on his DD Form 214.  Therefore, his DD Form 214 for the period ending 28 February 2013 should be corrected to show this award. 

2.  He was awarded 8 awards of the Army Good Conduct Medal.  His DD Form 214 listed only 7 awards.  Therefore, his DD Form 214 should be corrected to show the Army Good Conduct Medal (8th Award).

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from his DD Form 214 the Army Good Conduct Medal (7th Award) and adding the Purple Heart and Army Good Conduct Medal (8th Award).
