THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests change of item 19b (Nearest Relative) on his DD Form 214 (Certificate of Release or Discharge from Active Duty).
2. The applicant states the person listed is not a relative.
3. The applicant provides a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant served in the Regular Army from 31 December 2008 through 11 September 2011 and was separated due to a combat-related disability.
2. His DD Form 93 (Record of Emergency Data), dated 5 June 2010, lists the person shown in item 19b as his spouse who lived at the address shown on the DD Form 214.
3. Army Regulation 635-5 (Separation Documents) provided the guidance for completion of the DD Form 214. The directions for block 19a and 19b are: MAILING ADDRESS AFTER SEPARATION and NEAREST RELATIVE will be provided by the Soldier. This address must be a permanent address. Advise a Soldier that the name and address of a relative should be someone who will know their location and address at all times. When a relative is not available, use a close friend. When the person is at the same address as 19a enter (NAME) and ADDRESS SAME AS BLOCK 19a.
DISCUSSION AND CONCLUSIONS:
1. The available evidence shows that at the time the DD Form 214 was prepared the person listed in block 19b as his nearest relative was the applicant's spouse.
2. The applicant provided no evidence or argument that he meant to list some other person in block 19b.
3. There is no regulatory requirement to keep the DD Form 214 updated. It is not intended to be a running record, only a record of circumstances at the time of separation.
