      The applicant and counsel if any, did not appear before the Board.

      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 19a (Mailing Address after Separation)
of his DD Form 214 (Certificate of Release or Discharge from Active Duty)
be corrected to show an address in Tucson, AZ (the address listed in items
8 and 14 of his DD Form 149 (Application for Correction of Military Record
under the Provisions of Title 10, U. S. Code, Section 1552) dated 10
October 2003).

2.  The applicant states that the mailing address on his DD Form 214 is
incorrect. He completed a form prior to his discharge that showed his
correct address.

3.  The applicant provides his DD Form 214 and an undated Initial
Counseling Support Form.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 1 May 2003 from Phoenix,
AZ with a home of record of Tucson, AZ (not the same address he contends
was his mailing address).  His Statement of Personal History indicated both
his parents were alive and living in Carbon Hill, IL.

2.  The applicant completed an Initial Counseling Support Form, apparently
shortly after arriving at basic combat training.  On that form, he
indicated that his leave address after graduation would be the address he
now contends was his mailing address.  This is also his current address.

3.  While in basic combat training, the applicant was diagnosed with a
personality disorder.  Separation action was initiated and he was separated
on 14 August 2003.  He was not available to sign his DD Form 214.  Item 19
of his DD Form 214 shows a mailing address after separation of Los Angeles,
CA.  Item 19b (Nearest Relative (Name and Address)) of the DD Form 214
shows his nearest relative as Matthew L___ (not listed on his Statement of
Personal History as a relative) and the same address as entered in item
19a.

4.  Army Regulation 635-5 prescribes the separation documents prepared for
soldiers upon retirement, discharge, or release from active military
service or control of the Army.  It establishes standardized policy for the
preparation of the DD Form 214.  The instructions for preparing items 19a
and 19b state that the information will be provided by the soldier.  The
mailing address must be a permanent address.  When the relative named in
item 19b is at the same address as 19a, the entry, "(NAME) and ADDRESS SAME
AS BLOCK 19a" will be entered in 19b.  The information in these items is
used primarily when documents need to be forwarded to the soldier
immediately after separation.

DISCUSSION AND CONCLUSIONS:

1.  The information in item 19a of the DD Form 214 is used primarily when
documents need to be forwarded to the soldier immediately after separation.
 At this point in time, a change to that item would likely be of no
effective value to the applicant.

2.  However, it does appear that an error was made in item 19a of the
applicant's DD Form 214 and that he actually lived at the requested address
in Tucson, AZ at the time he separated.  He still lives at the requested
address.  He has been separated for less than one year.  It would therefore
still be appropriate to make the requested correction to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that item
19a of the DD Form 214 of the individual concerned be amended to show the
mailing address as it is listed in items 8 and 14 of his DD Form 149 dated
10 October 2003.
