THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the North Atlantic Treaty Organization (NATO) Medal and the Legion of Merit (LOM).

2.  The applicant states the supporting documentation was not available at the time her DD Form 214 was completed.

3.  The applicant provides:

* DD Form 214
* NATO Medal Certificate
* DA Form 4980-11 (The LOM)

CONSIDERATION OF EVIDENCE:

1.  The NATO Medal is shown on her DD Form 214C (Certificate of Release or Discharge from Active Duty (Continuation Sheet)).  No corrective action is necessary concerning this award; therefore, it will not be further addressed in these proceedings.

2.  The applicant enlisted in the Regular Army on 30 July 1986.  She honorably retired from active duty on 31 August 2013 in the rank of sergeant major and she was transferred to the Retired List on 1 September 2013.  The DD Form 214 she was issued does not show award of the LOM.  

3.  The applicant provides and her record contains Permanent Orders 112-1, issued by Headquarters, I Corps, Joint Base Lewis-McChord, WA, dated 22 April 2013, awarding her the LOM.

DISCUSSION AND CONCLUSIONS:

Permanent orders awarded the applicant the LOM which is not shown on her DD Form 214.  Therefore, she is entitled to correction of her DD Form 214 to show this award.
