THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Driver and Mechanic Badge and Meritorious Unit Commendation.

2.  The applicant states he has orders for both awards, but they are not listed on his DD Form 214.

3.  The applicant provides:

* U.S. Army Human Resources Command Permanent Orders 087-01
* Headquarters, 298th Combat Sustainment Support Battalion, Permanent Orders Number 112-34
* DD Form 214

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 2 June 2009.  He served as a motor transport operator in Afghanistan from 13 June 2010 to 12 June 2011 and he was released from active duty on 1 June 2012.

2.  His DD Form 214 does not show the Driver and Mechanic Badge or Meritorious Unit Commendation as authorized awards.

3.  He provided U.S. Army Human Resources Command Permanent 
Orders 087-01, dated 28 March 2011, which show Headquarters and Headquarters Company, 529th Support Battalion, and its subordinate units (including the 104th Transportation Company) was awarded the Meritorious Unit Commendation for the period 20 May 2010 to 15 March 2011 in Afghanistan.

4.  He provided Headquarters, 298th Combat Sustainment Support Battalion, Permanent Orders Number 112-34, dated 19 April 2011, which show he was awarded the Driver and Mechanic Badge for the period 26 November 2009 to 26 November 2010.  These orders show he was assigned to the 104th Transportation Company in Afghanistan.

DISCUSSION AND CONCLUSIONS:

1.  Permanent orders show the applicant was awarded the Driver and Mechanic Badge.  Therefore, his DD Form 214 should be corrected to show this badge.

2.  Permanent orders show his unit was awarded the Meritorious Unit Commendation while he was assigned to it.  Therefore, his DD Form 214 should be corrected to show this unit award.

adding award of the Meritorious Unit Commendation and Driver and Mechanic Badge to his DD Form 214.
