THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the: * Meritorious Service Medal * Army Good Conduct Medal (3rd Award) instead of Army Good Conduct Medal (2nd Award) 2. The applicant states the awards were not signed or approved prior to receiving his DD Form 214. 3. The applicant provides: * DD Form 214 * DA Form 638 (Recommendation for Award) * Meritorious Service Medal Certificate * 206th Military Intelligence Battalion Permanent Orders 098-001, dated 8 April 2014 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve under the Delayed Entry Program on 26 November 2003. On 18 April 2005, he was honorably discharged for immediate enlistment in the Regular Army. He enlisted in the Regular Army on 19 April 2005. 2. He provided a copy of his Meritorious Service Medal Certificate and DA Form 638 that shows Headquarters, U.S. Army Intelligence and Security Command, Permanent Orders 078-03, dated 19 March 2014, awarded him the Meritorious Service Medal. He also provided a copy of 206th Military Intelligence Battalion Permanent Orders 098-001, dated 8 April 2014, that awarded him the Army Good Conduct Medal (3rd Award) for the period 19 April 2011 to 18 April 2014. 3. On 7 June 2014, the applicant was honorably discharged. His DD Form 214 shows he was awarded or authorized the: * Afghanistan Campaign Medal with one bronze service star * Iraq Campaign Medal with three bronze service stars * Army Commendation Medal (4th Award) * Army Achievement Medal (2nd Award) * Meritorious Unit Commendation * Army Good Conduct Medal (2nd Award) * National Defense Service Medal * Global War on Terrorism Service Medal * Noncommissioned Officer Professional Development Ribbon * Army Service Ribbon * Overseas Service Ribbon (2nd Award) * NATO Medal * Senior Aircraft Crewman Badge (currently known as the Senior Aviation Badge) * Basic Aviation Badge * Driver and Mechanic Badge with Driver-W Clasp DISCUSSION AND CONCLUSIONS: The evidence of record shows permanent orders awarded the applicant the Meritorious Service Medal and Army Good Conduct Medal (3rd Award); therefore, he is entitled to correction of his DD Form 214 to show these awards.
