THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) by adding military education courses.

2.  The applicant states she completed the combat lifesaver, field sanitation, driver training and traffic safety courses.  She has all of the certificates as proof.

3.  The applicant provides a copy of a certificate of completion for the Combat Lifesaver Course comprised of 40 hours of instruction completed in September 2009.  No other certificates were provided.

CONSIDERATION OF EVIDENCE:

1.   The applicant entered active duty in the Regular Army on 10 August 2009.  She was discharged with a general discharge under honorable conditions on 22 November 2010.  

2.  Block 14 (Military Education) of her DD Form 214 shows only the Motor Transport Operator School/Course.

3.  The applicant's Interactive Personnel Electronic Records Management System (iPERMS) contains no Enlisted Record Brief (ERB) or evidence of training.

4.  Army Regulation 635-5 (Separation Documents) provides the guidance for completion of the DD Form 214.  The instructions for Block 14 are to list those courses shown on the ERB and include title, length in weeks, and year completed. 

DISCUSSION AND CONCLUSIONS:

1.  No ERB is available and, except for the certificate, it is unlikely that the applicant would have any record of attendance at a local course.  

2.  Therefore, it is in the interest of justice to add the Combat Lifesaver Course to her DD Form 214.

3.  The record did not reveal nor did the applicant provide sufficient evidence of completion of field sanitation, drivers training or traffic safety courses.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by
adding to block 14 of her DD Form 214, "Combat Lifesaving, 1 week, 2009." 

2.  The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to adding the field sanitation, drivers training or traffic safety courses.
