2. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal and five awards of the Army Achievement Medal. She requests that item
14 (Military Education) be corrected to show completion of the All Source Analysis System Remote Workstation Block II Training Course, the Driver Training Program and the Combat Lifesaver Course; and that a Certificate of Appreciation be shown on her DD Form 214. She also requests that her Defense Language Proficiency Test Scores in Spanish, an additional duty assignment and military occupational specialty (MOS) special duty assignment pay be shown on her DD Form 214.
3. The applicant states that this information is not shown on her current DD Form 214. In support of her application, she submits a copy of her DD Form 214; an award certificate for the Army Commendation Medal; a DA Form 638 (Recommendation for Award), dated 29 January 2002, for the Army Commendation Medal; an award certificate for the Army Achievement Medal; an undated DA Form 638 (Recommendation for Award) for a fifth award of the Army Achievement Medal; DA Form 330 (Language Proficiency Questionnaire), dated 18 September 2001; Defense language Proficiency Test Results, dated 28 April 2000; a Certificate of Training for the All Source Analysis System Remote Workstation Block II Training Course; a Certificate of Training for the Driver Training Program; a Certificate of Training for the Combat Lifesaver Course; a Certificate of Appreciation; a memorandum, dated 14 September 2000, for an additional duty assignment; and orders, dated 26 August 2000, for MOS special duty assignment pay.
4. The applicant’s military records show that she entered active duty on
20 August 1996. She served as an intelligence analyst and food service specialist and was honorably released from active duty on 24 March 2002 for completion of required active service.
5. The applicant’s DD Form 214 does not show the Army Commendation Medal or five awards of the Army Achievement Medal as authorized awards. Her DD Form 214 does show four awards of the Army Achievement Medal.
6. Item 14 (Military Education) on the applicant’s DD Form 214 does not show completion of the All Source Analysis System Remote Workstation Block II Training Course, the Driver Training Program or the Combat Lifesaver Course.
7. The applicant’s DD Form 214 does not show any certificates of appreciation, language proficiency test scores in Spanish, additional duty assignments or MOS special duty assignment pay.
8. The applicant provided an award certificate and DA Form 638 (Recommendation for Award) which shows that Permanent Order Number
070-006, dated 11 March 2002, awarded her the Army Commendation Medal for the period 5 May 2001 to 28 February 2002.
9. The applicant provided an undated DA Form 638 (Recommendation for Award) which shows that Permanent Order Number 037-003, dated 20 February 2002, awarded her the Army Achievement Medal for the period
4 September 2001 to 12 September 2001. Item 8 (Previous Awards) on this DA Form 638 shows four awards of the Army Achievement Medal.
10. The applicant provided a Certificate of Training which shows that she completed the All Source Analysis System Remote Workstation Block II Training Course during the period 16 July 2001 to 10 August 2001.
11. The applicant provided a Certificate of Training which shows that she completed the Driver Training Program during the period 4 - 8 August 1997.
12. The applicant provided a Certificate of Training which shows that she completed the Combat Lifesaver Course on 4 August 2000.
13. The applicant provided a Certificate of Appreciation, dated 20 June 2001, for outstanding support of the graduation ball for the Primary Leadership Development Course Class 7-01.
14. The applicant provided a DA Form 330 (Language Proficiency Questionnaire), dated 18 September 2001, and the results of her Defense Language Proficiency Test Scores in Spanish. Item 12 (Language Proficiency) on the applicant’s DA Form 2-1 (Personnel Qualification Record) shows her DA Form 330 (Language Proficiency Questionnaire) was submitted on 18 September 2001.
15. The applicant provided a memorandum, dated 14 September 2000, which shows that she was appointed an additional duty assignment as the “BOSS Representative” effective 14 September 2000. This entry is not shown in item
35 (Record of Assignments) on her DA Form 2-1.
16. The applicant provided orders, dated 26 August 2000, which show she was entitled to special duty assignment pay for Spanish language proficiency in military occupational specialty 96B (intelligence analyst) from 27 June 2000 to
26 June 2001. This entry is not shown in item 6 (Military Occupational Specialties) on her DA Form 2-1.
17. Army Regulation 635-5 establishes the policies and procedures for completion and distribution of the DD Form 214. In pertinent part it states that item 14 (Military Education) will list formal in service training courses
of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed. This information is to assist the soldier after separation in job placement and counseling; therefore, training courses for combat skills are not listed.
18. Army Regulation 635-5 provides that item 13 of the DD Form 214 will reflect decorations, medals, badges, citations and campaign ribbons awarded or authorized for all periods of service. Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214.
19. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. The regulation provides no authority to show language proficiency, additional duty assignments or MOS special duty assignment pay on the DD Form 214.
20. Army Regulation 600-8-104 (Military Personnel Information Management/Records) governs preparation of the DA Form 2-1 (Personnel Qualification Record). Chapter 5 of this regulation shows that language proficiency will be entered in item 12 (Language Proficiency) on the DA Form
2-1; that duty assignments will be entered in item 35 (Record of Assignments) on the DA Form 2-1; and that MOS’s will be entered in item 6 (Military Occupational Specialties) on the DA Form 2-1.
CONCLUSIONS:
1. The applicant received the Army Commendation Medal and the fifth award of the Army Achievement Medal. Therefore, she is entitled to correction of her DD Form 214 to show these awards.
2. The applicant completed the All Source Analysis System Remote Workstation Block II Training Course in 2001, the Driver Training Program in 1997 and the Combat Lifesaver Course in 2000. Therefore, she is entitled to correction of item 14 (Military Education) on her DD Form 214 to show these courses.
3. The Board considered the applicant’s request that a Certificate of Appreciation be shown on her DD Form 214. However, in accordance with the governing regulation, Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214. Therefore, there is no basis for granting the applicant’s request.
4. The Board also considered the applicant’s request that her language proficiency test scores in Spanish be shown on her DD Form 214. However, in accordance with Army Regulation 600-8-104, the applicant’s language proficiency in Spanish is properly annotated in item 12 on her DA Form 2-1. Therefore, there is no basis for granting the applicant’s request.
5. The applicant’s request to show an additional duty assignment and MOS special duty assignment pay on her DD Form 214 was also noted by the Board. However, there are no provisions for entering these items on the DD Form 214. Army Regulation 600-8-104 indicates that these items should be annotated on the DA Form 2-1. Therefore, there is no basis for granting the applicant’s request.
6. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records, but only as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected:
a. be deleting the entry, “ARMY ACHIEVEMENT MEDAL (4TH AWARD)” in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on the DD Form 214 of the individual concerned;
b. by adding the entries, “Army Commendation Medal” and “Army Achievement Medal (5th Award)” in item 13 on her DD Form 214; and
c. by adding the entries, “All Source Analysis System Remote Workstation Block II Training Course, 4 Weeks, 2001”, “Driver Training Program, 1 Week, 1997” and “Combat Lifesaver Course, 2000” in item 14 (Military Education) on her DD Form 214.
2. That so much of the application as is in excess of the foregoing be denied.
