THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of the middle initial shown on his Combat Action Badge orders.

2.  The applicant states the middle initial shown on the orders is not his.
 
3.  The applicant provides:

* his DD Form 214 (Certificate of Release or Discharge from Active Duty)
* Permanent Orders 276-027, issued by Headquarters, 143rd Sustainment Command (Expeditionary) Afghanistan, dated 3 October 2009

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 1 October 2008.  He was retired due to temporary disability effective 19 September 2011.  His DD Form 214 shows his middle name as "Vernon."  The Combat Action Badge is listed among awards listed on his DD Form 214.

2.  Permanent Orders 276-027, dated 3 October 2009, awarded the applicant the Combat Action Badge for service on 18 August 2009.  These orders show his middle initial as "B."


3.  His record contains an Enlisted Record Brief that shows his middle name as "Vernon."  It further shows in:

* Section I (Assignment Information) he served in Afghanistan from 14 April 2009 to 9 February 2010
* Section VIII (Awards and Decorations) the Combat Action Badge among other awards

DISCUSSION AND CONCLUSIONS:

1.  The available evidence shows his middle name as "Vernon"; therefore, his middle initial should be "V."  Permanent Orders 276-027, dated 3 October 2009, show his middle initial as "B."  That is obviously incorrect.

2.  In view of the foregoing, Permanent Orders 276-027, dated 3 October 2009, should be amended to corrected to reflect his middle initial as "V."
