THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that Headquarters, 3rd "Bronco" Brigade, 25th Infantry Division (Light), Permanent Order 200-01, dated 19 July 2005, for award of the Combat Medical Badge be amended by deleting his name. He also requests, in effect, that the Combat Medical Badge be removed from his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states, in effect, that the period of service shown on the Combat Medical Badge orders place him in Afghanistan before he arrived in Afghanistan. He contends that he deployed to Afghanistan in January 2005. 3. The applicant provides a copy of the orders in question; deployment orders; and a copy of his DD Form 214 in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant entered active duty on 13 September 2004. He served as a health services administrator in Afghanistan from 17 January 2005 to 24 April 2005 and served in Iraq from 27 July 2006 to 27 September 2007. On 13 September 2008, the applicant was honorably discharged for miscellaneous/general reasons in the rank of captain. 2. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 shows, among other awards, the Combat Medical Badge as an authorized award. 3. Headquarters, 3rd "Bronco" Brigade, 25th Infantry Division (Light), Permanent Order 200-01, dated 19 July 2005, shows the applicant received the Combat Medal Badge for exposure to enemy fire during the period 16 December 2004 to 19 December 2004 in Afghanistan. DISCUSSION AND CONCLUSIONS: 1. Evidence of record shows the applicant served in Afghanistan from 17 January 2005 to 24 April 2005. 2. Since the Combat Medical Badge orders erroneously state the applicant was in Afghanistan during the period 16 December 2004 to 19 December 2004, and based on the applicant's request, it would be appropriate to amend Headquarters, 3rd "Bronco" Brigade, 25th Infantry Division (Light) Permanent Order 200-01, dated 19 July 2005, by deleting the applicant's name. As a result of this correction, the Combat Medical Badge should be removed from his DD Form 214.
