IN THE CASE OF:
BOARD DATE: 16 August 2012
DOCKET NUMBER: AR20120003766
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "X0X-XX-XXXX."
2. The applicant states his SSN is "X0X-XX-XXXX."
3. The applicant provides:
* Social security card
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicants failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicants failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. The applicant enlisted in the Regular Army on 29 June 1965 for a period of
4 years. His DD Form 398 (Statement of Personal History), dated 7 July 1965, shows his SSN as "X0X-XX-XXXX." He served as a Morse interceptor and he was released from active duty on 27 June 1969 and transferred to the U.S. Army Reserve Control Group (Reinforcement) to complete his remaining service obligation.
3. Item 3 (SSN) of his DD Form 214 shows his SSN as "X2X-XX-XXXX."
4. He provides a copy of his social security card which shows his SSN as "X0X-XX-XXXX."
DISCUSSION AND CONCLUSIONS:
The applicant's DD Form 398, dated 7 July 1965, shows his SSN as "X0X-XX-XXXX." Therefore, it would be appropriate to correct his DD Form 214 to reflect his SSN as "X0X-XX-XXXX."
BOARD VOTE:
___X___ ___X____ ___X____ GRANT FULL RELIEF
________ ________ ________ GRANT PARTIAL RELIEF
________ ________ ________ GRANT FORMAL HEARING
________ ________ ________ DENY APPLICATION
BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the current SSN entry from his DD Form 214 and replacing it with the SSN shown on his social security card.

IN THE CASE OF: BOARD DATE: 20 December 2011 DOCKET NUMBER: AR20110012995 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. It appears that during the preparation of the applicant's DD Form 214 a typographical error occurred resulting in his SSN being recorded as x7x-xx-xxxx instead of the SSN as shown on his Social Security card. Therefore, it would be at this time to correct the applicant's DD Form 214 to show the SSN as shown on his Social Security card.
ARMY | BCMR | CY2011 | 20110014015
His military records show he was inducted into the Army on 28 July 1966. This DD Form 214 shows his SSN as X0X-XX-XXXX. Although all the documents in his records containing a SSN, with the exception of the DD Form 398, show his SSN as X0X-XX-XXXX, the DD Form 398 was completed by the applicant 2 days prior to his induction in the Army.
ARMY | BCMR | CY2012 | 20120002881
His DD Form 398 (Statement of Personal History) and DD Form 4 (Enlistment Contract) both show his SSN as "x3x-xx-xxxx." Item 3 (SSN) of the DD Form 214 he was issued at the time shows his SSN as "x0x-xx-xxxx." As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from item 3 of his DD Form 214 the current entry and replacing it with the social security number as shown on his social security card.

The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to clearly show his last name as "D----" and his Social Security Number (SSN) as "xx9-xx-xxxx" instead of "xx3-xx-xxxx." It states that the complete name and SSN of the separating service member would be entered on the DD Form 214. Therefore, it would be appropriate at this time to correct his DD Form 214 accordingly.
