THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service in Iraq and the Iraq Campaign Medal with one campaign star.

2.  He states item 12f (Foreign Service) of his DD Form 214 should show his foreign service during the period 10 May 2007 through 10 July 2008 and his Iraq Campaign Medal should show one campaign star instead of two. 

3.  He provides his DD Form 214. 

CONSIDERATION OF EVIDENCE:

1.  His military records show he enlisted in the Regular Army on 29 December 2004. 

2.  He was honorably released from active duty on 28 March 2012 in the rank of sergeant/E5 and transferred to the U.S. Army Reserve Control Group (Reinforcement) to complete his remaining Reserve obligation.  He completed 7 years and 3 months of creditable active service.

3.  The DD Form 214 he was issued at the time is void of an entry in item 12f.  This form further shows:

* he served in Iraq during the period 10 May 2007 through 10 July 2008 
(1 year, 2 months, and 1 day)
* he was authorized the Iraq Campaign Medal with two campaign stars

4.  Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF).  The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles.  The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to 31 December 2011.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Appendix B shows participation credit was awarded for the relevant campaign of Iraqi Surge (10 January 2007-31 December 2008).

5.  Army Regulation 635-5 (Separation Documents) states a DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty service.  It provides a brief, clear-cut record of active service at the time of release from active duty, retirement, or discharge.  It states enter  in block 12f the total amount of foreign service completed during the period covered in block 12c (Net Active Service This Period).

DISCUSSION AND CONCLUSIONS:

1.  His DD Form 214 shows he served in Iraq for 1 year, 2 months, and 1 day.  As such, he is entitled to have his DD Form 214 amended so show "010201" in item 12f.

2.  Evidence shows he was authorized the Iraq Campaign Medal and that he participated in the Iraqi Surge campaign.  As such, it would be appropriate to amend his DD Form 214 to show the Iraq Campaign Medal with one campaign star.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to show:

   a.  "010201" in item 12f and
   
   b.  the Iraq Campaign Medal with one bronze service star in lieu of the two “campaign stars” now listed.
