THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests the record of her late husband, a former service member (FSM), be corrected to show the second digit of his social security number (SSN) as "6 instead of "5."
2. The applicant states her supporting documents reflect the proper SSN.
3. The applicant provides the following documents in support of her application:
* SSN printout, dated 21 December 2011
* Marriage Certificate
* Department of Defense Uniform Services Identification and Privilege Card (ID Card)
* Certificate of Death, dated 9 December 2011
* DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge), dated 5 June 1969
CONSIDERATION OF EVIDENCE:
1. The FSM enlisted in the Regular Army on 22 May 1964. The record contains a DD Form 398 (Statement of Personal History) prepared by the FSM during his enlistment processing. This document lists the second digit of his SSN as 6 in item 13 (Employment).
2. Item 9 (SSN) of the FSM's DA Form 41 (Record of Emergency Data) dated 25 June 1968 lists the second digit of his SSN as 6.
3. Item 32 (Remarks) of the FSM's DD Form 214 dated 27 May 1965 lists the second digit of his SSN as 6.
4. Item 3 (SSN) of the FSM's DD Form 214 dated 5 June 1969 lists the second digit of his SSN as 5 instead of 6.
5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge,
or release from active duty service or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. The regulation in effect at the time of the applicant's separation stated documents on file in the MPRJ were the primary sources for information for entries on the DD Form 214.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record confirms the FSM entered active duty and he served under the SSN using the number 6 as the second digit as listed on his DD Form 398, DA Form 41, and the 1965 DD Form 214, as corroborated and confirmed by the SSN printout provided by the applicant. As a result, it appears the SSN entry on the FSM's 1969 DD Form 214 was the result of an administrative error.
2. In view of the foregoing, it would be appropriate at this time to correct this DD Form 214 as recommended below.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending item 3 of his DD Form 214 for the period ending 5 June 1969 by replacing the second digit of the SSN as "6" instead of "5" as shown on his previous DD Form 214.

The FSM completed a DD Form 398 (Statement of Personal History) in connection with his induction into the Army of the United States (AUS). He recorded his SSN as 180-XX-XXXX. The preponderance of the documents in the FSM's Army Military Human Resource Record show his SSN as 130-XX-XXX; however, his DA Form 41 (Record of Emergency Data) also shows his SSN as 180-XX-XXXX.

In connection with his induction into the Army of the United States (AUS), the FSM completed a DD Form 398 (Statement of Personal History) wherein he listed his DOB as 25 July 1946. His DD Form 47 (Record of Induction) listed his DOB as 24 July 1946 and his Service Number. The evidence of record shows upon his induction into the AUS, the FSM disclosed his DOB as 25 July 1946.
