THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) of her deceased father, a former service member (FSM), be corrected to show his social security number (SSN) as "XXX-XX-XX-9664" instead of "XXX-XX-9644." 

2.  The applicant states the SSN listed on her father's DD Form 214 is incorrect.  This error may be affecting entitlement to benefits.

3.  The applicant provides:

* Her birth certificate
* FSM's DD Form 214
* FSM's death certificate
* FSM's social security card
* FSM's driver license
* FSM's DA Form 20 (Enlisted Qualification Record)

CONSIDERATION OF EVIDENCE:

1.  In connection with his induction into the Army of the United States (AUS), the FSM completed a DD Form 398 (Statement of Personal History) and listed his SSN as "XXX-XX-9664." 

2.  He was inducted into the AUS on 7 November 1963.  His DD Form 47 (Record of Induction) lists his service number; it does not list his SSN.  
3.  His DA Form 24 (Service Record) and subsequent DA Form 20, which were created upon his entry on active duty, listed his SSN as "XXX-XX-9664."

4.  He was honorably released from active duty on 6 May 1966.  Item 32 (Remarks) of his DD Form 214 shows his SSN as "XXX-XX-9644."

5.  The applicant provides a copy of the FSM's social security card that shows his SSN as "XXX-XX-9664."

6.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation states that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service.  It is important that information entered on the form should be complete and accurate.

DISCUSSION AND CONCLUSIONS:

1.  The FSM listed his SSN with the last four digits as "9664" upon his induction into the AUS.  This SSN is consistent with the SSN shown on multiple documents in his service records listing his SSN.  It appears that when his DD Form 214 was created, it inadvertently listed the last four digits as "9644" instead of "9664." 

2.  For historical purposes, the U.S. Army has an interest in maintaining the accuracy of its records.  In this regard, the information contained therein should reflect the conditions and circumstances that existed at the time the records were created and under which the military service was performed.  Therefore, he is entitled to the requested relief.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
* deleting from item 32 of his DD Form 214 the entry"XXX-XX-9644"
* adding to item 32 of his DD Form 214 the entry "XXX-XX-9664” as shown on his social security card
