THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) of her deceased spouse, a former service member (FSM), to show his social security number (SSN) as
180-XX-XXXX instead of 130-XX-XXXX.
2. The applicant makes no additional statement.
3. The applicant provides the FSM's:
* DD Form 214
* death certificate
* social security card
* driver license
CONSIDERATION OF EVIDENCE:
1. The FSM completed a DD Form 398 (Statement of Personal History) in connection with his induction into the Army of the United States (AUS). He recorded his SSN as 180-XX-XXXX.
2. He was inducted into the AUS on 23 May 1966. His DD Form 47 (Record of Induction) lists his service number; it does not list his SSN.
3. The preponderance of the documents in the FSM's Army Military Human Resource Record show his SSN as 130-XX-XXX; however, his DA Form 41 (Record of Emergency Data) also shows his SSN as 180-XX-XXXX.
4. The FSM was honorably released from active duty on 17 May 1968. Item 3 (SSN) of his DD Form 214 shows his SSN as 130-XX-XXXX.
5. The applicant provides a copy of the FSM's social security card that shows his SSN as 180-XX-XXXX.
6. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. This regulation states the purpose of the separation document is to provide the individual with documentary evidence of his or her military service. It is important that information entered on the form should be complete and accurate.
DISCUSSION AND CONCLUSIONS:
1. The FSM recorded his SSN as 180-XX-XXXX in conjunction with his induction into the AUS. It appears that the second digit was inadvertently recorded as "3" instead of "8" at some point after his induction. It appears that the SSN shown on his DD Form 214 as 130-XX-XXXX is in error.
2. For historical purposes, the U.S. Army has an interest in maintaining the accuracy of its records. In this regard, the information contained therein should reflect the conditions and circumstances that existed at the time the records were created and under which the military service was performed. Therefore, the applicant is entitled to the requested relief.

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the current SSN from the FSM's DD Form 214 and replacing it with the SSN shown on his social security card.
