THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release of Discharge from Active Duty) to show his campaign medal received for his service performed in support of Operation Iraqi Freedom (OIF).
2. The applicant states the award he earned for his active duty service completed in support of OIF was omitted from his DD Form 214.
3. The applicant provides a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant's military record shows that after having prior military service, he enlisted in the Oregon Army National Guard (ORARNG) on 2 April 1991. He was ordered to active duty in support of OIF on 2 May 2009.
2. His official military personnel file (OMPF) contains U.S. Army Human Resources Command, Permanent Orders 211-01 dated 30 July 2010, which announced the award of the Meritorious Unit Commendation (MUC) to the
41st Infantry Brigade Combat Team and its subordinate units, for service in support of OIF. Company C, 186th Infantry Regiment, the applicant's unit, was listed on this order and his name was listed on the manifest accompanying this order.
3. On 19 December 2011, the applicant was retired from active duty by reason of permanent disability. The DD Form 214 issued at that time shows he served in Iraq from 2 May to 29 August 2009 and that he earned the following awards:
* U.S. Marine Corps Good Conduct Medal
* Army Reserve Components Achievement Medal
* National Defense Service medal with bronze service star
* Global War on Terrorism Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Armed Forces Reserve Medal with M Device
* U.S. Navy Sea Service Deployment Ribbon
4. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 December 2011)
DISCUSSION AND CONCLUSIONS:
1. Records show that the applicant served during a qualifying period that authorizes him the Iraq Campaign Medal with one bronze serve star. Therefore, this medal should be shown on his DD Form 214.
2. Orders show the applicant's unit was awarded the MUC for service in support of OIF. Therefore, it would be appropriate to add the MUC to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214, issued on 19 December 2011, by:
a. adding the Meritorious Unit Commendation and Iraq Campaign Medal with one bronze service star; and
b. issuing him a correction to his DD Form 214 to reflect this change.
