THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 26 January 2012 to show a second award of the Meritorious Service Medal.

2.  She states her current DD Form 214 does not include her second award of the Meritorious Service Medal.  

3.  She provides her DD Form 214 for the period ending 26 January 2012, a DA Form 638 (Recommendation for Award), and an award certificate for the Meritorious Service Medal.

CONSIDERATION OF EVIDENCE:

1.  At the time the applicant submitted her application, she was serving in the active Army Reserve in the rank of lieutenant colonel.  

2.  After completion of prior enlisted service in the U.S. Navy and U.S. Navy Reserve, on 2 September 1993, she was appointed as a Reserve commissioned officer in the Army Nurse Corps.  

3.  On 8 June 2006, she was ordered to active duty in support of Operation Iraqi Freedom (OIF).  She served in Iraq from 28 September 2006 to 23 October 2007.

4.  Her service record contains an undated award certificate which indicates she was awarded the Meritorious Service Medal by Task Force 3rd Medical Command, Permanent Order Number 163-030, for meritorious service from 16 October 2006 to 16 September 2007.  

5.  She was released from active duty on 23 October 2007.  Her DD Form 214 for the period ending 23 October 2007 shows, in part, one award of the Meritorious Service Medal.

6.  On 3 November 2010, she was ordered to active duty again in support of OIF.  She served in Iraq from 6 December 2010 to 30 October 2011.

7.  She provided a copy of a DA Form 638, dated 6 July 2011, and an award certificate, dated 30 July 2011, which indicate she was awarded a second award of the Meritorious Service Medal by the Commander, U.S. Forces-Iraq, Baghdad, Iraq, on Permanent Order Number 211-040, dated 30 July 2011 for meritorious service from 3 November 2010 to 20 October 2011.

8.  She was released from active duty on 26 January 2012.  Her DD Form 214 for the period ending 26 January 2012 shows, in part, one award of the Meritorious Service Medal.  

DISCUSSION AND CONCLUSIONS:

1.  Permanent orders awarded the applicant the Meritorious Service Medal for meritorious service during the period 16 October 2006 to 16 September 2007.  This award is listed on her DD Forms 214 for the periods ending 23 October 2007 and 26 January 2012.  

2.  Permanent orders awarded the applicant a second award of the Meritorious Service Medal for meritorious service during the period 3 November 2010 to 20 October 2011.  

3.  However, her DD Form 214 for the period ending 26 January 2012 shows only one award of the Meritorious Service Medal.  Therefore, it would be appropriate to amend her DD Form 214 for the period ending 26 January 2012 to show a second award of the Meritorious Service Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 for the period ending 26 January 2012 by:

	a.  deleting the Meritorious Service Medal; and

	b.  adding the Meritorious Service Medal (2nd Award).
