THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests her second deployment to Iraq from 1 January 2008 to 
2 August 2008 be entered on her DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 31 December 2010.  She also requests an additional Army Commendation Medal and Meritorious Service Medal be added to her DD Form 214.

2.  The applicant states the second deployment and awards are missing from her DD Form 214.

3.  The applicant provides:

* her orders to deploy in support of Operation Iraqi Freedom (OIF)
* a letter, dated 18 July 2008, from Medical Task Force 66, Baghdad, Iraq
* two DA Forms 638 (Recommendation for Awards) with attached documents

CONSIDERATION OF EVIDENCE:

1.  She retired on 31 December 2010 and was placed on the Retired List the following day.  Item 13 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) of her DD Form 214 shows she was awarded or is authorized the:

* Meritorious Service Medal
* Army Commendation Medal (5th award)
* Army Achievement Medal
* National Defense Service Medal (2nd award)
* Global War on Terrorism Service Medal
* Korea Defense Service Medal
* Iraq Campaign Medal with bronze service star
* Army Service Ribbon
* Overseas Service Ribbon (4th award)

2.  Item 18 (Remarks) contains the entry "SERVICE IN IRAQ 20050331 - 20051007 (2005 March 31 - 2005 October 07)."

3.  U.S. Army Garrison, Fort George G. Meade, Fort George G. Meade, MD Orders 344-25, dated 10 December 2007, ordered her to deploy in support of OIF.  She was ordered to proceed 1 January 2008 to Fort Benning, GA for processing.

4.  A memorandum, dated 8 July 2008, from Medical Task Force 66, Baghdad, Iraq, Subject: Statement of Service, states she was deployed in Iraq from 
27 January to 2 August 2008.

5.  The Defense Finance and Accounting Service (DFAS) confirmed she received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) for Kuwait from 27 January through 2 August 2008.

6.  A DA Form 638, dated 3 June 2008, approved her for the award of the Army Commendation Medal under Permanent Order Number 153-06.

7.  Her official military personnel file (OMPF) shows five previous order numbers for the award of the Army Commendation Medal.

8.  A DA form 638, dated 28 July 2010, approved her for the award of the Meritorious Service Medal under Permanent Order Number 265-02, dated 
23 September 2010.

9.  Her OMPF shows one previous order number for the award of the Meritorious Service Medal.

10.  Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of OIF.  The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles.  The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Approved campaigns include:

* Iraqi Governance (29 June 2004 - 15 December 2005)
* Iraqi Surge (10 January 2007 - 31 December 2008)

11.  Army Regulation 635-5 (Separation Documents) provides that when an active duty Soldier is deployed with his unit during the period covered by the DD Form 214, an entry will be made in item 18 to show the name of the country deployed to and the inclusive dates in YYYY/MM/DD format.

DISCUSSION AND CONCLUSIONS:

1.  The information received from DFAS and the letter from Task Force 66 are sufficient to show her second deployment to Iraq was from 27 January 2008 to 
2 August 2008 (20080127 - 20080802).  Therefore, the deployment entry on her DD Form 214 should be amended to show IRAQ 20050331 - 20051007 and 20080127 - 20080802.

2.  The DA Form 638, dated  June 2008, approved her for the award of the Army Commendation Medal.  She was previously awarded five awards of the Army Commendation Medal.  Therefore, item 13 of her DD Form 214 should be amended to show she was awarded the Army Commendation Medal (6th award).

3.  The DA Form 638, 28 July 2010, approved her for the award of the Meritorious Service Medal.  She was previously awarded the Meritorious Service Medal.  Therefore, item 13 of her DD Form 214 should be amended to show she was awarded the Meritorious Service Medal (2nd award).

4.  Based on both of her tours of duty in Iraq, she is authorized two bronze service stars to be worn on her Iraq Campaign Medal.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 as follows:

	a.  Item 13 - amend to show she was awarded the:

* Meritorious Service Medal (2nd award)
* Army Commendation Medal (6th award)

	b.  Item 18 - amend to read SERVICE IN IRAQ 20050331 - 20051007 and 20080127 - 20080802.

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to any change to her second tour in Iraq in excess of the above.
