THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his record to show the Bronze Star Medal (BSM), Army Commendation Medal (ARCOM) (4th Award), and North Atlantic Treaty Organization (NATO) Medal.
2. The applicant states his end of tour BSM and NATO Medal from Afghanistan were lost in the mail and not received until after his DD Form 214 (Certificate of Release or Discharge from Active Duty) was finalized and he was separated from service.
3. The applicant provides copies of:
* two DA Forms 638 (Recommendation of Award)
* three award certificates
* BSM Narrative Summary and Citation
CONSIDERATION OF EVIDENCE:
1. The applicant's records show he was appointed a Reserve commissioned officer in the rank of second lieutenant (2LT/O-1), in the Field Artillery Branch, on 31 May 2003. He was ordered to active duty in the Regular Army on 13 June 2003.
2. His official military personnel file (OMPF) contains three ARCOM certificates awarded between 28 November 2003 and 18 December 2006. Each certificate identifies the official order that announced these awards. 3. On 1 July 2006, the applicant was promoted to captain (CPT/O-3). He continuously served until he was honorably discharged from active duty on 1 March 2011, having completed 7 years, 8 months, and 19 days of creditable active duty service of which 2 years, 11 months, and 5 days was foreign service.
3. The DD Form 214 issued him at that time shows he earned the following awards:
* Afghanistan Campaign Medal with bronze service star
* ARCOM (3rd Award)
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with bronze service star
* Army Service Ribbon
* Overseas Service Ribbon (3rd Award)
* Combat Action Badge
* Parachutist Badge
* Air Assault Badge
4. He provides:
a. A DA Form 638, which shows United States Forces Afghanistan South, published Permanent Orders Number (#) 229-011, dated 3 September 2010, awarding him the BSM for exceptional meritorious service for the period 15 August 2009 through 14 August 2010.
b. A DA Form 638, which shows Headquarters, 4th Brigade, Fort Bragg, North Carolina, published Permanent Orders #068-047, dated 10 March 2011, awarding him the ARCOM with 3rd Oak Leaf Cluster (OLC), for exceptional meritorious service for the period 1 July 2008 through 31 March 2011.
c. A certificate awarding him the NATO Medal for the period 15 August 2009 through 14 August 2010. (This certificate is also included in his OMPF.)
DISCUSSION AND CONCLUSIONS:
1. The applicant contends his DD Form 214 should be corrected to show he earned the BSM, ARCOM (4th Award), and NATO Medal).
2. The evidence in this case includes permanent orders which awarded him the BSM and ARCOM with 3rd OLC. His OMPF contains the certificate that announced his award of the NATO Medal. None of these awards are currently shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 to show these awards.
3. In view of the above, his request should be granted.

a. amending his DD Form 214 by deleting the Army Commendation Medal (3rd Award) and adding the following awards:
* Bronze Star Medal
* Army Commendation Medal (4th Award)
* NATO Medal
b. issuing him a correction to his DD Form 214 to reflects this correction.
