THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 1 June 2010 to show:
* Item 4a (Grade, Rate or Rank) as sergeant first class (SFC) vice staff sergeant (SSG)
* Item 4b (Pay Grade) as E-7 vice E-6
* Item 11 (Primary Specialty) as 21N4O, Construction Equipment Operator - 10 years, 9 months; 74D3O, Chemical Operations Specialist - 1 year, 2 months; 89B40, Ammunition Specialist - 16 years, 11 months vice 74D3O, Chemical Operations Specialist - 1 year, 2 months
2. The applicant states he tried to bring up the errors at the time he was out-processing; however, the personnel assistant would not look at any other documents. He was directed to sign his DD Form 214 and to submit corrections at a later date.
3. The applicant provides three DD Forms 214 and two orders.
CONSIDERATION OF EVIDENCE:
1. The applicant's records show he enlisted in the Regular Army (RA) on 1 October 1992 and he held military occupational specialty (MOS) 55B (Ammunition Specialist) now known as MOS 89B.
2. He was honorably released from the RA on 30 September 1996 in the rank of specialist and transferred to the U.S. Army Reserve (USAR). He completed 4 years of creditable active service during this period of service.
3. Item 11 of the DD Form 214 he was issued for this period of service shows the entry "55B1O 00, Ammunition Specialist - 3 years and 7 months."
4. He was ordered to active duty as a member of his USAR unit in support of Operation Enduring Freedom and entered active duty on 7 December 2003. He was promoted to the rank of SSG on 15 March 2004.
5. He was honorably released from active duty on 11 November 2004 and assigned to his USAR unit. He completed 11 months and 11 days of creditable active service during this period of service.
6. Item 11 of the DD Form 214 he was issued for this period of service shows the entries "21E3O, Heavy Construction Equipment Operator - 5 years and 2 months" and "55B3O, Ammunition Specialist - 11 years and 4 months."
7. He was ordered to active duty as a member of his USAR unit in support of Operation Iraqi Freedom and entered active duty on 16 September 2009.
8. Orders Number 10-112-00009, dated 22 April 2010, issued by Headquarters, 81st Regional Support Command, Fort Jackson, SC, promoted him to the rank of SFC effective 1 April 2010. This order also awarded him primary MOS (PMOS) 21N4O (Construction Equipment Supervisor), secondary MOS (SMOS) 74D4O, and additional MOS (AMOS) 89B4O, effective 1 April 2010.
9. He was honorably released from active duty on 1 June 2010 and assigned to his USAR unit. He completed 1 year, 1 month, and 16 days of creditable active service during this period of service.
10. The DD Form 214 he was issued for this period of service shows the following in:
* Items 6a and 6b, "SSG" and "E-6"
* Item 11, "74D3O 00, Chemical Operations Specialist - 1 year and 2 months"
* Item 12i (Effective Date of Pay Grade) "2004 03 15"
* Item 21b (Date) shows the applicant signed the form on 1 April 2010
* Item 22b (Date) show the authorized official signed the form on 1 April 2010
11. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states in item 11 of the DD Form 214 enter the titles of all MOS served for at least 1 year and the number of years and months served. For time determination, 16 days or more count as a month.
DISCUSSION AND CONCLUSIONS:
The evidence of record shows that effective 1 April 2010 the applicant was promoted to SFC and awarded PMOS 21N4O, SMOS 74D4O, and AMOS 89B4O, which are not shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 for the period ending 1 June 2010 to show the correct information.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 for the period ending 1 June 2010 as follows in:
* Item 4a delete the entry and replace with SFC
* Item 4b delete the entry and replace it with E-7
* Item 11 delete the entry and replace it with:
* 21N4O 00, Construction Equipment Supervisor - 10 years and 9 months
* 74D4O 00, Chemical Operations Specialist 1 year and 2 months
* 89B4O 00, Ammunition Specialist - 16 years and 11 months
* Item 12i delete the entry and replace it with 2010 04 01
