THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect both of his deployments and his correct amount of foreign service.
2. The applicant states that his DD Form 214 only reflects one of his deployments and only 1 year of foreign service. However, he deployed twice to Iraq and served 2 years of foreign service.
3. The applicant provides a copy of his DD Form 214, memoranda from his commander depicting his deployments, and copies of awards received during his deployments.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 3 February 2005 for a period of 4 years and training as a health care specialist. He completed his training and was transferred to Fort Hood, Texas for his first and only duty assignment.
2. He deployed to Iraq from 20051211 20061209 (11 months and 29 days) and was advanced to the pay grade of E-4 on 1 October 2006.
3. He again deployed to Iraq from 20080310 20090307 (11 months and 28 days) and was returned to Fort Hood where he remained until he was honorably released from active duty (REFRAD) on 15 July 2009 due to completion of required service.
4. His DD Form 214 issued at the time of his REFRAD shows in block 12f (Foreign Service) that he served 1 year of foreign service, and block 18 (Remarks) reflects only one deployment, during the period 20080310 20090309.
5. The applicant served 1 year, 11 months, and 26 days of foreign service during his two deployments.
6. Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for:
a. block 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214.
b. block 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. The applicants contention that his DD Form 214 incorrectly reflects only one tour in Iraq and only 1 year of foreign service has been noted and found to have merit.
2. The applicant deployed to Iraq from 20051211 20061209 and 20080310 20090307 for a total of 1 year, 11 months, and 26 days of foreign service. Accordingly, his DD Form 214 should be corrected to reflect that information.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
* deleting the entry "0001 00 00" in block 12f of his DD Form 214
* adding the entry "0001 11 26 days" to block 12f of his DD Form 214
* deleting the entry "SERVICE IN IRAQ 20080310-20090309" from
block 18 of his DD Form 214
* adding the entry "SERVICE IN IRAQ FROM 20051211-20061209 AND 20080310-20090307" to block 18 of his DD Form 214
2. The Board wants the applicant and all others to know the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated. The applicant and all Americans should be justifiably proud of his service in arms.
