THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to reflect his service in Afghanistan.
2. The applicant states he served in Afghanistan from March 2012 to 8 August 2012.
3. The applicant provides his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 17 July 2008. He served in Iraq from 6 August 2009 to 12 June 2012 (10 months and 7 days).
2. Orders Number LS-082-0001, issued by Installation Management Command, Joint Base Lewis-McChord on 22 March 2012, ordered his deployment to Afghanistan in support of Operation Enduring Freedom (OEF), beginning on or about 15 March 2012.
3. The Defense Finance and Accounting Service (DFAS) confirmed he received hostile fire pay/imminent danger pay (HFP/IDP) and combat zone tax exclusion (CZTE) during the period 26 March - 20 August 2012 (4 months and 26 days).
4. He was honorably released from active duty on 12 November 2012. His DD Form 214 shows in block 12f (Foreign Service) the entry "0000 10 07," which correlates to his service in Iraq. His DD Form 214 does not show awards authorized for service in Afghanistan in support of OEF, and it does not reflect the dates of his service in Afghanistan in block 18 (Remarks).
5. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of OEF. The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are:
* Liberation of Afghanistan (11 September 2001-30 November 2001)
* Consolidation I (1 December 2001-30 September 2006)
* Consolidation II (1 October 2006-30 November 2009)
* Consolidation III (1 December 2009 30 June 2011)
* Transition I (1 July 2011-date to be determined)
6. Army Regulation 635-5 (Separations Documents), in effect during the applicant's active duty service, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It established standardized policy for the preparation of the DD Form 214. It stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. The regulation provided that for:
a. block 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214;
b. block 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (name of country deployed) FROM (inclusive dates, for example, YYYYMMDD-YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that his DD Form 214 should be corrected to reflect his service in Afghanistan has been carefully considered.
2. Orders show he was ordered to deploy to Afghanistan in support of OEF with a departure date of on or about 15 March 2012. DFAS records confirm he received HFP/IDP and CZTE for during the period 26 March - 20 August 2012 (4 months and 26 days). Therefore, his DD Form 214 should be corrected to reflect the additional amount of foreign service performed in block 12f and to document his deployment to Afghanistan in block 18.
3. Based on his service in Afghanistan, he is authorized the Afghanistan Campaign Medal with one bronze service star and correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the current entry in block 12f of his DD Form 214 and adding the entry "0001 03 03;
b. adding to his DD Form 214 the Afghanistan Campaign Medal with one bronze service star; and
c. adding to block 18 of his DD Form 214 the entry "SERVICE IN AFGHANISTAN FROM 20120326-20120820."
