THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his deployment in Iraq. He also requests award of the Iraq Campaign Medal. 2. The applicant states, in effect, that his deployment in Iraq is not shown on his DD Form 214. 3. The applicant provides a Soldier Deployment History Outprocessing Report in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 11 June 2002. He served as a health care specialist and was released from active duty on 4 March 2008. 2. The applicant’s DD Form 214 does not show the Iraq Campaign Medal as an authorized award. Item 12f (Foreign Service) on his DD Form 214 shows the entry, "000 00 00." His DD Form 214 does not show any deployments in item 18 (Remarks). 3. The applicant provided a Soldier Deployment History Outprocessing Report which shows he served in a Non-U.S. worldwide location from 1 August 2006 to 1 September 2007 (396 days). 4. The applicant's DA Form 2166-8 (Noncomissioned Officer Evaluation Report) for the period 1 July 2006 through 30 June 2007 shows that his unit was stationed in Iraq and that he earned the Combat Medical Badge during Operation Iraqi Freedom for exemplary performance of duty while coming under enemy attack. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that, for block 12 (Record of Service) f (Foreign Service), from the Enlisted Record Brief, enter the total amount of foreign service completed during the period covered in block 12c (Net Active Service Period this Period). 6. Army Regulation 635-5 further states that, for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in block 18 (Remarks)of the DD Form 214. 7. Army Regulation 600-8-22 (Military Awards) states that individuals authorized the Iraq Campaign Medal must have served in direct support of Operation IRAQI FREEDOM (OIF). The area of eligibility encompasses all land area of the country of Iraq, and the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. A bronze service star is authorized for the following campaigns: Liberation of Iraq 19 March 2003 – 1 May 2003; Transition of Iraq 2 May 2003 – 28 June 2004; Iraqi Governance 29 June 2004 – 15 December 2005; and National Resolution 16 December 2005 – Date to be determined. DISCUSSION AND CONCLUSIONS: 1. The entries on the Soldier Deployment History Outprocessing Report which shows the applicant was deployed to Iraq from 1 August 2006 to 1 September 2007 and the applicant’s DA Form 2166-8 are accepted as sufficient evidence on which to amend items 12f and 18 on his DD Form 214. 2. Based on his service in Iraq, the applicant is eligible for award of the Iraq Campaign Medal with one bronze service star.
