THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be amended to show additional awards of the Army Commendation Medal and the Army Achievement Medal.

2.  The applicant states she received the awards after her discharge date.  She states that her documents were not available to be included on the original DD Form 214.

3.  The applicant provides a copy of her DD Form 214, two DA Forms 638 (Recommendation for Award), her DA Form 71 (Oath of Office), and two award certificates.  

CONSIDERATION OF EVIDENCE:

1.  The applicant is currently serving on active duty as a Regular Army second lieutenant in the Medical Service Corps.  

2.  The applicant enlisted in the Regular Army on 27 June 1995.  She continued to serve on active duty through two reenlistments until she was discharged on 31 December 2008 to accept a commission or warrant in the Army.  

3.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of her DD Form 214 shows one award of the Army Commendation Medal and the Army Achievement Medal (3rd Award).

4.  The applicant provided a DA Form 638 and certificate which shows she was awarded a fourth award of the Army Achievement Medal for meritorious achievement from 1 December to 12 December 2007 by Brooke Army Medical Center, Fort Sam Houston, TX Permanent Order Number 005-03, dated 
5 January 2009.

5.  The applicant also provided a DA Form 638 and certificate which shows she was awarded a second award of the Army Commendation Medal for meritorious service from 10 August 2007 to 9 January 2009 by Brooke Army Medical Center, Fort Sam Houston, TX Permanent Order Number 014-04, dated 14 January 2009.

6.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  In pertinent part it states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.

DISCUSSION AND CONCLUSIONS:

1.  Permanent orders show the applicant was awarded a second award of the Army Commendation Medal for meritorious service from 10 August 2007 to 9 January 2009.  Therefore, the second award of the Army Commendation Medal should be recorded on her DD Form 214 for the period ending 31 December 2008.  

2.  Permanent orders show the applicant was awarded a fourth award of the Army Achievement Medal for the period 1 December 2007 to 12 December 2007. Therefore, her DD Form 214 should be amended to add a fourth award of the Army Achievement Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the entry "Army Achievement Medal (3rd Award)" from item 13 of her DD Form 214 and replacing it with the entry "Army Achievement Medal 
(4th Award)"; and

	b.  deleting the entry "Army Commendation Medal" from item 13 of her DD Form 214 and replacing it with the entry "Army Commendation Medal (2nd Award)."
