THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was released from active duty (REFRAD) on 17 May 2010 instead of 17 May 2009 in block 12b (Separation Date This Period).
2. The applicant states that his DD Form 214 incorrectly reflects that he was REFRAD on 17 May 2009 instead of 17 May 2010.
3. The applicant provides a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant was serving in the Oregon Army National Guard in pay grade
E-4 when he was ordered to active duty in support of Operation Iraqi Freedom on 2 May 2009. He deployed to Iraq from 27 June 2009 to 13 April 2010.
2. On 17 May 2010, he was honorably REFRAD at Fort Lewis, Washington, due to completion of required service. He completed 1 year and 16 days of net active service during this period. At the time of his REFRAD, his DD Form 214 was prepared to show he was REFRAD on 17 May 2009, which is only 15 days after he was ordered to active duty.
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that his DD Form 214 reflects an incorrect REFRAD date has been noted and was found to have merit.
2. The applicant was ordered to active duty on 2 May 2009 and he served 1 year and 16 days of active service before being REFRAD. However, his DD Form 214 incorrectly shows he was REFRAD on 17 May 2009 instead of 17 May 2010.
3. Accordingly, his DD Form 214 should be corrected to show he was REFRAD on "2010 05 17" in block 12b.

* adding the entry "2010 05 17" to block 12b of his DD Form 214
