THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her foreign service in Africa and to recalculate her foreign service.

2.  The applicant states she served in Africa during the period 17 June 2007 to 17 July 2007 and this should be reflected on her DD Form 214 in item 12f (Foreign Service) and item 18 (Remarks).

3.  The applicant provides:

* her DD Form 214
* her deployment orders
* her DD Form 1351-2 (Travel Voucher or Subvoucher)

CONSIDERATION OF EVIDENCE:

1.  The applicant is a U.S. Army Reserve major.  Her DD Form 214 shows she served on active duty from 5 January 2004 through 31 December 2008.

2.  Orders Number 149-352, U.S. Army Combined Arms Center and Fort Leavenworth, Fort Leavenworth, KS, dated 29 May 2007, directed the applicant to proceed on 9 June 2007 and return not later than 31 July 2007 to:

* Horn of Africa
* Iraq

* Afghanistan
* and other areas of U.S. Central Command

3.  The applicant's travel voucher is for the period 9 June 2007 through 17 July 2007 and shows she traveled to:

* Djibouti
* Uganda

4.  During the applicant's active duty service, her DD Form 214 shows she performed foreign service as follows:

* Kuwait/Iraq – 20051206-20060810
* Kuwait/Iraq – 20070609-20070731
* Kuwait/Iraq – 20071014-20071103
* Kuwait/Iraq – 20081014-20081019

DISCUSSION AND CONCLUSIONS:

1.  It appears the applicant's service in Africa was recorded on her DD Form 214 as travel to Kuwait/Iraq during the period 20070609-20070731.  Not only is the location in error, but the end date should be reflected as 17 July 2007 per her travel voucher.

2.  The applicant's DD Form 214 should be corrected as follows:

* change the foreign service in item 12f –

* from "0000  11  03"
* to "0000  10  11"

* delete the entry "SERVICE IN KUWAIT/IRAQ 20070609-20070731" from item 18
* add the entry "SERVICE IN DJIBOUTI/UGANDA FROM 20070609-20070717" to item 18

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 to:

* delete "0000  11  03" from item 12f
* add "0000  10  11" to item 12f
* delete "SERVICE IN KUWAIT/IRAQ 20070609-20060731" from item 18
* add "SERVICE IN DJIBOUTI/UGANDA FROM 20070609-20070717" to item 18
