      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of her discharge
document.

2.  The applicant states, in effect, that her discharge document is in
error and should be corrected to show her service in Iraq and the awards
that she is authorized for that service.

3.  The applicant provides a copy of Defense Finance and Accounting Service
(DFAS), Travel Voucher Number 546486, dated 25 March 2004; Headquarters,
21st Combat Support Hospital, Fort Hood, Texas memorandum for record, dated
4 June 2004; and her DD Form 214 (Certificate of Release or Discharge from
Active Duty), with an effective date of 26 July 2004.

CONSIDERATION OF EVIDENCE:

1.  The applicant's military service records contain DD Form 4 series
(Enlistment/ Reenlistment Document - Armed Forces of the United States),
dated  26 July 1999, which shows she enlisted in the U.S. Army Reserve for
a period of 8 years on 26 July 1999.  Section F (Discharge from Delayed
Entry/Enlistment Program) of the applicant's DD Form 4/3 shows that on 27
July 2000 she requested discharge from the Delayed Entry/Enlistment Program
and enlistment in the Regular Army for a period of 4 years.  Section H
(Confirmation of Enlistment or Reenlistment) of this document shows that
the applicant confirmed her enlistment in the Regular Army on 27 July 2000
by signing the document in Item 22b (Signature of Enlistee/Reenlistee).

2.  The applicant’s record shows that, upon entry on active duty, she was
trained in and served in military occupational specialty (MOS) 91W (Health
Care Specialist) and was assigned to the 21st Combat Support Hospital, Fort
Hood, Texas.  The highest rank the applicant attained while serving on
active duty was specialist/pay grade E-4.  She was honorably released from
active duty on
26 July 2004 after serving on active duty for a total of 4 years.

3.  The applicant's DD Form 214, Block 12 (Record of Service), Item 12f
(Foreign Service) contains the entry "0000 00 00".  Item 13 (Decorations,
Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of
this document shows that she was awarded the Global War on Terrorism
Service Medal, National Defense Service Medal, Army Service Ribbon, and the
Army Lapel Button.  Item 18 (Remarks) is absent an entry showing the
applicant deployed overseas to a foreign country or that she was in the
Delayed Entry/Enlistment Program.
4.  The applicant provides a copy of her DD Form 214, DFAS Travel Voucher,
and a memorandum from the captain in command of the 21st Combat Support
Hospital (Fort Hood) in support of her application.  The applicant's travel
voucher shows that she was paid for temporary duty location of Kuwait
(Other) from
24 March 2003 to 4 February 2004.  The memorandum from the applicant's
commander shows that she was deployed with her unit in Mosul, Iraq, from
23 March 2003 to 1 February 2004.

5.  A review of the applicant's record reveals that she may be entitled to
additional awards that are not shown on her DD Form 214.

6.  There is no evidence the applicant received the first award of the Good
Conduct Medal.  There also is no evidence the applicant was disqualified by
her chain of command from receiving the Good Conduct Medal and her records
do not contain any adverse information.

7.  There is no evidence the applicant was awarded the Iraq Campaign Medal
for her service in Iraq in support of Operation Iraqi Freedom from 24 March
2003 to 4 February 2004.

8.  There is no evidence the applicant was awarded the Overseas Service
Ribbon for completion of an overseas tour in Iraq.

9.  Army Regulation 600-8-22 (Military Awards) provides that the Good
Conduct Medal is awarded to individuals who distinguish themselves by their
conduct, efficiency and fidelity during a qualifying period of active duty
enlisted service.  This period is 3 years except in those cases when the
period for the first award ends with the termination of a period of Federal
military service.  Although there is no automatic entitlement to the Good
Conduct Medal, disqualification must be justified.

10.  U.S. Army Human Resources Command, Military Awards Branch message,
dated 8 April 2005, provided the following implementing instructions for
award of the Iraq Campaign Medal.  Award of the Iraq Campaign Medal is
authorized for service in direct support of Operation Iraqi Freedom on or
after 19 March 2003 to a future date to be determined by the Secretary of
Defense or the cessation of the operation.  The area of eligibility
encompasses all land area of the country of Iraq and the contiguous sea
area out to 12 nautical miles and the air space above these areas.  To
qualify for award of this campaign medal, service members must have been
assigned or attached to or mobilized with units operating in these areas of
eligibility for 30 consecutive days or for 60 non-consecutive days or meet
one of the following criteria:  a) be engaged in combat during an armed
engagement, regardless of the time in the area of eligibility;
b) be medically evacuated from the area of eligibility for wounds or
injuries resulting from participation in an operation or official duties;
or c)  participates as a regularly assigned air crewmember flying sorties
into, out of, within or over the area of eligibility in direct support of
the military operations [each day of operations counts as one day of
eligibility].  This campaign medal may be awarded posthumously.

11.  Army Regulation 600-8-22 shows that the Overseas Service Ribbon was
established by the Secretary of the Army on 10 April 1981.  The regulation
states, in pertinent part, that effective 1 August 1981, all members of the
Active Army, Army National Guard, and Army Reserve in an active Reserve
status are eligible for the award for successful completion of overseas
tours.  The award may be awarded retroactively to those personnel who were
credited with a normal overseas tour completion before 1 August 1981
provided they had an Active Army status on or after 1 August 1981.  On 25
October 2005, a change to the requirements for award of the Overseas
Service Ribbon eliminated the policy that restricted award of the Overseas
Service Ribbon when another U.S. campaign/service medal is awarded.

12.  Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that must be prepared for Soldiers on retirement, discharge,
release from active duty service, or control of the Active Army.  It also
establishes standardized policy for preparing and distributing the DD Form
214.  Chapter 2 contains guidance on the preparation of the DD Form 214.
It states, in pertinent part, that the source documents for entering
information on the DD Form 214 will be the Personnel Qualification Record
(PQR), Enlisted/Officer Record Brief (ERB/ORB), or any other document
authorized for filing in the OMPF.

13.  Paragraph 2-4 of the separation documents regulation contains item-by-
item instructions for completing the DD Form 214.  The instructions for
Item 12f state, in pertinent part, to enter the total amount of foreign
service completed during the period covered in Item 12c.  The instructions
for completing Item 18 state, in pertinent part, for an active duty Soldier
deployed with his or her unit during their continuous period of active
service enter the statement, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM
(inclusive dates for example, YYYYMMDD - YYYYMMDD)".  In addition, the
instructions for completing Item 18 state, in pertinent part, for a Soldier
who has Delayed Entry Program (DEP) time, enter "BLOCK 6 - PERIOD OF DEP
(inclusive periods of DEP time)".  Finally, the
instructions for Item 13 show that the awards entered will be for all
periods of service and instructs the preparing official to check the
Soldier's service records for the validity of awards.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends, in effect, that her DD Form 214 should be
corrected because it does not show her service in Iraq or the awards she is
authorized for her service.

2.  The evidence of records shows that the applicant entered the U.S. Army
Reserve Delayed Entry/Enlistment Program in an inactive status on 26 July
1999 and was discharged from the U.S. Army Reserve for immediate entry in
the Regular Army in an active duty status on 27 July 2000.  Therefore, it
is appropriate to correct her DD Form 214 to show this time in the Delayed
Entry/ Enlistment Program.

3.  The evidence of records shows that the applicant was deployed overseas
in Iraq in support of Operation Iraqi Freedom from 24 March 2003 to 4
February 2004.  This period of foreign service equates to 10 months and 11
days.  Therefore, Item 12f of the applicant's DD Form 214 should be
corrected to show the entry "0000 10 11".  In addition, Item 18 of the
applicant's DD Form 214 should also be corrected with an entry to show
overseas service in Iraq from
24 March 2003 to 4 February 2004.

4.  The applicant is entitled to the first award of the Good Conduct Medal
for the period 27 July 2000 through 26 July 2003 based on completion of a
period of qualifying active duty enlisted service.  Therefore, she is
entitled to correction of her records to show this award.

5.  The evidence of record shows that the applicant qualified and met the
criteria for award of the Iraq Campaign Medal.  Therefore, it would be
appropriate to correct her records to show this service medal.

6.  The evidence of record shows that the applicant qualified and met the
criteria for award of the Overseas Service Ribbon.  Therefore, it would be
appropriate to correct her records to show this service medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
making the following corrections to the applicant's DD Form 214:

      a.  Item 12f.  Foreign Service
                 Delete:  "0000   00  00"
                 Add:     "0000  10   11"

      b.  Item 13.  Decorations, Medals, Badges, Citations and Campaign
Ribbons Awarded or Authorized
