THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he served in Qatar for 6 days from 16 to 22 November 2008.

2.  The applicant states he served in Qatar on temporary duty (TDY) from 16 to 22 November 2008 and he would like an entry on his DD Form 214 to show this service.

3.  The applicant provides:

* his DD Form 214 for the period ending 30 April 2011
* a DD Form 1610 (Request and Authorization for TDY Travel of DOD Personnel), dated 6 October 2008
* his DD Form 1351-2 (Travel Voucher or Sub-voucher) and travel settlement
* his Defense Finance and Accounting Service (DFAS) Leave and Earnings Statement (LES) for the period 1-31 December 2008

CONSIDERATION OF EVIDENCE:

1.  The applicant is a retired Regular Army Medical Services Corps officer who initially entered active duty on 2 January 1985.  He served in a variety of stateside or overseas assignments and he attained the rank of colonel (COL).

2.  On 6 October 2009, while assigned to Fort Sam Houston, TX, he was issued TDY orders to attend a conference (Central Command Surgeon's Conference in Doha, Qatar.  He departed Fort Sam Houston, TX, and he arrived in Qatar on   16 November 2008.  He then departed Qatar on 21 November 2008 and he arrived at Fort Sam Houston on 22 November 2008.

3.  His December 2008 LES shows he received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) for the month of November 2008.

4.  He was honorably retired on 30 April 2011 and he was placed on the retired list in his retired rank of COL on 1 May 2011.  His DD Form 214 shows he completed 26 years, 3 months, and 29 days of creditable active service.
Block 18 (Remarks) does not show an entry for his travel to Qatar.

5.  DFAS confirmed the applicant received HF/IDP and CZTE for service in Qatar from 16 to 21 November 2008.

6.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  Chapter 2 contains guidance on the preparation of the DD Form 214.  It states for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter in block 18 the statement "Service in (Name of Country Deployed) From (inclusive dates for example, YYYYMMDD - YYYYMMDD)"

DISCUSSION AND CONCLUSIONS:

The applicant contends he served in Qatar from 16 to 21 November 2008.  His TDY orders and travel settlement voucher confirm this travel.  Additionally, DFAS confirmed he received HF/IDP and CZTE for service in Qatar from 16 to 21 November 2008.  Therefore, he is entitled to correction of his DD Form 214 to show his service in Qatar.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to block 18 of his DD Form 214 the entry "Service in Qatar from 20081116 - 20081121."
