THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


CONSIDERATION OF ADDITIONAL EVIDENCE:

8.  On 22 February 2011, new information was received from the applicant in regard to the decision of the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20090009364, dated 5 November 2009.

9.  The evidence submitted consists of a statement from the applicant wherein he contends that the correction recommended and subsequently made in the Record of Proceedings (ROP) was not properly executed in accordance with the documentary evidence and request made by the applicant.  The applicant also provides copies of his letter orders for discharge, request for discharge, social security card, and his Honorable Discharge Certificate showing the improper correction.

10.  The available evidence clearly shows that the last four numbers of the applicant's social security number should read 5941.

ADDITIONAL DISCUSSION AND CONCLUSIONS:

2.  At the time of the decision of the ABCMR in Docket Number AR20090009364, dated 5 November 2009, it was the intent of the ABCMR to make the applicant’s record as administratively correct as it should properly have been at the time.

3.  The ABCMR's decision in Docket Number AR20090009364 failed to recognize that two portions of the applicant's social security number were incorrect.  The ROP only directed correction of the middle two numbers, overlooking the reversal error of the center two numbers of the last portion of the social security number.

4.  The available evidence clearly shows that the last four numbers of the applicant's social security number should read 5941, and not 5491, as incorrectly shown on his Honorable Discharge Certificate.

The Board determined that the evidence presented was sufficient to warrant amendment of the decision of the ABCMR set forth in Docket Number AR20090009364, dated 5 November 2009, to show that the applicant's correct social security number is xxx-xx-5941, and that he should be issued a corrected Honorable Discharge Certificate showing this social security number.
