THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of items 12a (Date Entered AD [Active Duty] This Period) and 12c (Net Active Service This Period) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 9 December 2009. 2. The applicant states he attended the Basic Officer Leadership Course (BOLC) II from 6 July 2009 through 20 August 2009 and this active duty service is not reflected on his DD Form 214. 3. The applicant provides his: * DD Form 214 for the period ending 9 December 2009 * Fort Bragg (FB) Form 1059 (BOLC II Evaluation Report) * BOLC Diploma CONSIDERATION OF EVIDENCE: 1. The applicant was appointed as a second lieutenant in the U.S. Army Reserve on 18 January 2009. 2. FB Form 1059 shows he attended the BOLC II Course during the period 6 July 2009 through 20 August 2009 and he achieved course standards. 3. On 9 December 2009, the applicant was released from active duty. 4. Item 12a of the applicant's DD Form 214 for the period ending 9 December 2009 shows the entry "2009 08 22" [22 August 2009]. Item 12b (Separation Date This Period) shows the entry "2009 12 09" [9 December 2009]. Item 12c shows the entry "0000 03 18" [3 months and 18 days]. 5. The applicant's retirement points detail statement shows he was ordered to active duty for training from 6 July 2009 through 9 December 2009. 6. Records from the Defense Finance and Accounting Service (DFAS) show the applicant was on active duty from 6 July 2009 through 9 December 2009. 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part, it states that the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. The regulation states, in pertinent part, that for item 12 (Record of Service) use extreme care in completing this block since post-service benefits, final pay, retirement credit, and so forth are based on this information. It further states that for: a. item 12a, enter the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued; b. item 12b, enter the Soldier’s transition date; and c. item 12c, enter the amount of service this period by subtracting item 12a from 12b. DISCUSSION AND CONCLUSIONS: The applicant's retirement points detail statement and DFAS information which show he was ordered to active duty for training from 6 July 2009 through 9 December 2009, for a period of 6 months and 4 days, is accepted as sufficient evidence in which to correct items 12a and 12c of his DD Form 214 for the period ending 9 December 2009.
