THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of Item 7b (Home of Record at Time of Entry) of her DD Form 214 (Certificate of Release or Discharge from Active Duty), for the period ending 1 August 2010, to show "Mililani, Hawaii" instead of "1xxxx H------ P---, Fort Jackson, South Carolina."

2.  She states she transferred from the U.S. Air Force to the Regular Army at Fort Jackson just prior to starting the Captain’s Career Course.  Her home of record when entering military service was Hawaii.  She had no break in service.

3.  She provides a copy of her DD Form 214 for her period of service in the U.S. Air Force.

CONSIDERATION OF EVIDENCE:

1.  The applicant's military records show she was initially appointed in the U.S. Air Force Reserve (USAFR) from the Air Force Reserve Officers' Training Corps (University of Hawaii), as a second lieutenant, effective 6 July 2002.  She entered active duty on 6 August 2002.

2.  Army Human Resources Command, Office of Appointments, St. Louis, MO, issued Orders A-09-624726, dated 27 September 2006, appointing her in the U.S. Army Reserve (USAR), as a second lieutenant, with an effective date of
15 October 2006, with concurrent order to active duty for 3 years.  The orders specified she would report on active duty as a result of her inter-service transfer 

to the U.S. Army from the U.S. Air Force.  The orders also advised she was to report to Fort Jackson.  The orders further indicate that her home of record is the same as her SNL (standard name line), which was Fort Jackson.

3.  She was honorably released from the U.S. Air Force on 31 October 2006 for an intra-departmental transfer.  Item 7b of her U.S. Air Force DD Form 214 shows Mililani, Hawaii, as her home of record.

4.  She executed a DA Form 71 (Oath of Office - Military Personnel) for appointment in the Regular Army on 1 November 2008 at Fort Jackson, and she entered active duty on 8 November 2006, also at Fort Jackson.

5.  She served in various staff and leadership positions and she was honorably discharged from the Regular Army on 1 August 2010.  She apparently accepted appointment in the Army National Guard and was assigned to a unit in Hawaii.  Item 7b of her DD Form 214 shows "Fort Jackson, South Carolina" as her home of record.

6.  She was appointed as a captain in the Hawaii Army National Guard, effective 2 August 2010.

7.  The Joint Travel Regulation defines an individual's home of record as the place recorded when commissioned, appointed, enlisted, inducted, or ordered into a tour of active duty.  The Joint Travel Regulation serves as the authority for correcting a home of record that was erroneously entered at the time of entry on active duty.  It states, in pertinent part, that a correction must be based on evidence that a bona fide error was made at the time of entry into the relevant period of service.  It must not be a place selected for the convenience of the Soldier.

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record shows her home of record at the time of entry on active duty was Mililani, Hawaii.

2.  The evidence shows she was appointed in the USAR with an active duty reporting date prior to her release from the U.S. Air Force based on an inter-service transfer.  Although she was ordered to active duty at Fort Jackson, her DD Form 214 should show her home of record as Mililani, Hawaii since she had no break of service and she did not request a change to her home of record.  Therefore, she is entitled to correction of Item 7b of her DD Form 214 for the period ending 1 August 2010 to show Mililani, Hawaii.

3.  Based on the correction to Item 7b of her DD Form 214 for the period ending 1 August 2010, it would also be appropriate to amend Army Human Resources Command, St Louis, Orders A-09-624726, dated 27 September 2006, to show her home of record as Mililani, Hawaii.

4.  In view of the foregoing, the applicant’s records should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

   a.  deleting from Item 7b of her DD Form 214 for the period ending 1 August 2010 the entry "1xxxx H------ P---, Fort Jackson, South Carolina 29207," and replacing it with the entry "Mililani, Hawaii"; and

   b.  amending Army H uman Resources Command, Orders A-09-624726, dated 27 September 2006, to change her home from "same as SNL" to "Mililani, Hawaii."
