THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his home of record (HOR) as Pohnpei FM 96941 instead of Pohnpei Hawaii 96941.
2. The applicant states the clerk made a mistake when generating his DD Form 214.
3. The applicant provides a Department of the Army (DA) memorandum, his DD Form 4 (Enlistment/Reenlistment Document, Armed Forces of the United States), and DA orders.
CONSIDERATION OF EVIDENCE:
1. The applicant's record shows he enlisted in the Regular Army at Pearl Harbor, HI for a period of 3 years on 3 November 2009. Item 3 (HOR) of his DD Form 4 shows his HOR as "Pohnpei, FM 96941." The applicant authenticated this form by placing his signature in the appropriate block.
2. On 16 December 2009, he was discharged in accordance with Army Regulation 635-200 (Personnel Separations) paragraph 5-11, by reason of failed medical/physical/procurement standards. Item 7b (Home of Record at the Time of Entry) of the DD Form 214, he was issued shows the entry "Pohnpei, Hawaii 96941."
3. The applicant provides a memorandum from Headquarters (HQ), U.S. Army Garrison Command, Chief, Transition Center, Fort Knox, KY, dated 3 May 2010, to the National Personnel Records Center, St Louis, MO, requesting, in pertinent part, correction of his (the applicant's) DD Form 214 as follows:
* Item 7b, from "Pohnpei, Hawaii 96941" to Pohnpei, FM 96941"
* Item 19a (Mailing Address After Separation), from "Pohnpei, Hawaii 96941" to "Pohnpei, FM 96941"
* Item 19b (Nearest Relative) from "Kolonia, Hawaii 96941" to "Pohnpei, FM 96941"
4. There is no indication the noted corrections were accomplished as requested.
5. He also provides DA Orders 123-0173, issued by HQ, U.S. Army Garrison Command, Fort Knox, KY, dated 3 May 2010, amending his separation orders as follows: As reads: "HOR: Pohnpei, HI, US" to "HOR: Pohnpei, Micronesia (FM).
6. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. This regulation specified that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. The DD Form 214 is prepared for all personnel at the time of their retirement, discharge, or release from active duty. Item 7b shows the city, state and ZIP code listed as the Soldiers HOR. Home of Record is the place recorded as the HOR of the Soldier when commissioned, appointed, enlisted, or ordered to a tour of active duty.
DISCUSSION AND CONCLUSIONS:
1. The applicants contention that his home of record on his DD Form 214 should be corrected to show Pohnpei, FM was carefully considered and found to have merit.
2. The evidence of record contains his enlistment contract showing his HOR as Pohnpei, FM at the time he entered active duty. He also provides a memorandum requesting correction of his DD Form 214 to reflect his HOR, mailing address after separation, and address of nearest relative as Pohnpei, FM. Therefore, his DD Form 214 should be corrected to show these corrections.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from item 7b of his DD Form 214 the entry "*** POST OFFICE BOX, POHNPEI HAWAII 96941" and replacing it with the entry "*** POST OFFICE BOX, POHNPEI FM 96941";
b. deleting from item 19a Of his DD Form 214 the entry "*** POST OFFICE BOX, POHNPEI HAWAII 96941" and replacing it with the entry "*** POST OFFICE BOX, POHNPEI FM 96941"; and
c. deleting from item 19b of his DD Form 214 the entry "E*** N. H*****, *** POST OFFICE BOX, KOLONIA HAWAII 96941" and replacing it with the entry "E*** N. H*****, *** POST OFFICE BOX, POHNPEI FM 96941."
