	The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that the home of record (HOR) on his DD Form 
214 with the period ending 7 November 2006 be corrected to show ____ ________ _______, McDonald, PA, 15057.

2.  The applicant states, in effect, that item 7b (Home of Record at Time of Entry), 19a (Mailing Address after Separation), and 19b (Nearest Relative) of his DD Form 214 incorrectly show ____ ________ _______, Indiana, PA, 15705-0001. 

3.  The applicant provides his Pennsylvania Driver's License that was issued to him on 29 May 2003.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the U.S. Army Reserve (USAR) on 17 May 2003.  His DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) showed his HOR as ____ ________ _______, McDonald, PA, 15057.

2.  U. S. Army Reserve Officers' Training Corps (ROTC) Battalion, Indiana University of Pennsylvania Orders 133-4, dated 14 May 2006, show that the applicant was being discharged from the USAR to accept a commission in the U.S. Armed Forces.  The address on the orders shows ____ ________ _______, McDonald, PA, 15057.

3.  The applicant was appointed as a Reserve Commissioned Officer of the U.S. Army in the grade of Second Lieutenant on 13 May 2006.  His appointment memorandum shows his address as ____ ________ _______, McDonald, PA, 15057.  He was issued a PA driver’s license in 2003 showing his address as 
____ ________ _______, McDonald, PA, 15057.

4.  Headquarters, U.S. Army Reserve Command (USARC) Orders M06-152-061, dated 1 June 2006, erroneously show the applicant's address in the standard name line as ____ ________ _______, Indiana, PA, 15705-0001 and his HOR as Indiana, PA, 15705-0001 (it should read ____ ________ _______, McDonald, PA, 15057).  

5.  A DD Form 214, with the ending period 7 November 2006, shows the applicant entered active duty training (ADT) on 4 June 2006 and was released from ADT on 7 November 2006.  Items 7b, 19a, and 19b erroneously show the entries ____ ________ _______, Indiana, PA, 15705-0001 (they should have ____ ________ _______, McDonald, PA, 15057).
6.  Army Regulation 600-8-104 prescribes instructions for preparing the Personnel Qualification Records, to include the Officer Record Brief (ORB).  In pertinent part it states that the source document for the home of record entry on the ORB will be the active duty letter or order.

DISCUSSION AND CONCLUSIONS:

The applicant's DD Form 214 with the period ending 7 November 2006 and USARC Orders M06-152-061, dated 1 June 2006 erroneously show his HOR as ____ ________ _______, Indiana, PA, 15705-0001.  The applicant's DD Form 4/1, Pennsylvania Driver's License, and appointment orders confirm that his DD Form 214 and USARC Orders M06-152-061 are incorrect and should be corrected to show the HOR as ____ ________ _______, McDonald, PA, 15057.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that the applicant's DD Form 214 with the period ending 11 November 2006 be corrected to show ____ ________ _______, McDonald, PA, 15057 in item 7b, item 19a, and item 19b; and to amend the HOR on Headquarters, USARC Orders M06-152-061, dated 1 June 2006 to read ____ ________ _______, McDonald, PA, 15057.
