THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 23 January 2009 to show award of the Army Achievement Medal (AAM) and a second award of the North Atlantic Treaty Organization (NATO) Medal (correctly known as the NATO Medal with one bronze service star). 

2.  The applicant states:

* he received the AAM during his deployment but he did not have the orders  in hand at the time of his demobilization for input by out-processing personnel
* he served under multiple commands while in theater and throughout his mobilization
* since he was an individual member attached to incoming units he did not demobilize with the units he received the AAM with and he obtained the orders after he returned home
* he received two awards of the NATO Medal, one for his initial mobilization period (combat area he served in qualified for the "Balkans" NATO Medal) and one for another area and command he served under as an extended volunteer
* his DD Form 214 for the period ending 23 January 2009 shows one award of the NATO Medal



3.  The applicant provides:

* two applications
* orders for the AAM
* memorandum for award of the Non-Article 5 NATO Medal (Balkans)
* DD Form 214 for the period ending 23 January 2009
* DD Form 214 for the period ending 26 March 1991
* DD Form 215 (Correction to DD Form 214)  

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty from the U.S. Army Reserve on 
6 December 1990 in support of Operation Desert Shield/Desert Storm.  He was released from active duty on 26 March 1991.  

2.  The applicant was ordered to active duty from the Army National Guard on 
28 July 2006 in support of Operation Enduring Freedom.  He served in Kosovo from 17 October 2006 to 21 November 2008.  On 23 January 2009, he was released from active duty.  

3.  The applicant's DD Form 214 for the period ending 23 January 2009 does not show the AAM as an authorized award.  This DD Form 214 does show, among other awards, the NATO Medal with one bronze service star (i.e., two awards of the NATO Medal).  Since this DD Form 214 shows two awards of the NATO Medal, this portion of his request will not be discussed further in these proceedings.   

4.  Headquarters, Multi-National Task Force (East) Permanent Order Number 190-005, dated 8 July 2008, show the applicant received the AAM for the period 21 September 2007 to 10 July 2008. 

DISCUSSION AND CONCLUSIONS:

Orders show the applicant received the AAM.  Therefore, his DD Form 214 for the period ending 23 January 2009 should be corrected to show this medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the AAM on his DD Form 214 for the period ending 23 January 2009.
