THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of that Item 12a (Date Entered Active Duty This Period) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show 17 June 2008 instead of 17 June 2009.

2.  The applicant states that the entry date on her DD Form 214 indicates she enlisted on 17 June 2009, but she actually enlisted on 17 June 2008.

3.  The applicant provides the following documents in support of her application: discharge orders, dated 1 July 2009; enlistment orders, dated 17 June 2008; DD Form 214 for the period ending 8 July 2008; DD Form 4/3 (Enlistment/
Reenlistment Document), dated 17 June 2008; and Enlisted Record Brief (ERB).

CONSIDERATION OF EVIDENCE:

1.  The applicant’s DD Form 4/3 shows she enlisted in the Regular Army (RA) on 17 June 2008 for a period of three years.

2.  Department of Defense, Military Entrance Processing Station, Cleveland, OH Orders 8169008, dated 17 June 2008, shows the applicant enlisted in the U.S. Army and she was assigned to the 120th Army Adjutant General Battalion (Reception), Fort Jackson, SC, with a reporting date of 17 June 2008.

3.  The applicant’s ERB shows she was assigned to the 120th Army Adjutant General Battalion (Reception), Fort Jackson, on 17 June 2008.
4.  Headquarters, U.S. Army Basic Combat Training Center of Excellence, Fort Jackson, Orders 182-1318, dated 1 July 2009, shows the applicant was discharged on 8 July 2009.

5.  Her DD Form 214 shows she entered active duty on 17 June 2009 and she was discharged on 8 July 2009, which equates to 22 days of net active service.  Her DD Form 214 shows she completed 1 year and 6 days of net active service this period.

6.  Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214.  It states that the DD Form 214 is a synopsis of the soldier's most recent period of continuous active duty.  It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.  In pertinent part, it directs that the beginning date of the continuous period of active duty will be entered in Item 12a for issuance of this DD Form 214, for which a DD Form 214 was not previously issued.  It further directs that for Item 12c, enter the amount of service this period, computed by subtracting Item 12a from Item 12b (Separation Date This Period), less lost time, if any.

DISCUSSION AND CONCLUSIONS:

1.  Orders published by the Department of Defense, Military Entrance Processing Station, Cleveland, and the applicant’s DD Form 4/3 show she enlisted in the RA on 17 June 2008.  She was discharged on 8 July 2009.  However, her DD Form 214 erroneously reflects she entered active duty on 17 June 2009 and she completed 1 year and 6 days of active military service.

2.  It appears that an administrative error has occurred in this case and it would be appropriate to amend Item 12a on the applicant's DD Form 214 to show she entered active duty on 17 June 2008 and completed 1 year and 22 days of active military service.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting from Item 12a of her DD Form 214 the entry “2009 06 17” and replacing it with the entry “2008 06 17”; and

	b. deleting from Item 12c (Net Active Service This Period) of her DD Form 214 the entry "0001 00 06" and replacing it with the entry "0001 00 22."
