THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction to her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the correct date she entered active duty (AD). 2. She states item 12a (Date Entered AD This Period) is incorrect as shown. It should show: 2005 10 26. 3. She provided: * a DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) * a letter * her Enlisted Record Brief (ERB) * her DD Form 214 CONSIDERATION OF EVIDENCE: 1. Her DD Form 4 shows she enlisted in the U.S. Army Reserve for 8 years in the Delayed Entry/Enlistment Program (DEP) on 26 August 2005. Item 8a (For Enlistment in a DEP) shows the entry 20060105 which indicates she was scheduled to enter the Regular Army on 5 January 2006. She was released from DEP status on 25 October 2005 and enlisted in the Regular Army on 26 October 2005. 2. She provided an ERB which shows in Section III (Service Data) her Basic Active Service Date (BASD) as 26 October 2005. Section IX (Assignment Information) shows, in pertinent part, she was assigned to the Reception Company, 120th Adjutant General Battalion, Fort Jackson, SC on 26 October 2005. 3. She also provided a letter, addressed to the family members of Soldiers scheduled to graduate the basic combat training class, on 20 January 2006. This document provides instructions to family members desiring to attend the graduation. 4. On 1 May 2008, she was honorably discharged. Her DD Form 214 contains the following entries: * Item 12a – 2006 01 05 * Item 12c (Net Active Service This Period) – 0002 03 27 * Item 18 (Remarks) – PERIOD OF DEP: 20050826 - 20060104 5. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. It is important that information entered on the form should be complete and accurate. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that: a. Item 12a shows the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued; b. Item 12b (Separation Date This Period) shows the Soldier’s transition date; c. Item 12c (Net Active Service This Period) shows the total amount of active service for that period. It is computed by subtracting item 12a from item 12b; and d. Item 18 (Remarks) contains Headquarters, Department of the Army mandatory requirements when a separate block is not available, and as a continuation for other entries. DEP time that began on or after 1 January 1985 will be entered in this item. DISCUSSION AND CONCLUSIONS: 1. It is apparent an administrative error occurred when preparing her DD Form 214. The date entered in item 8a of her DD Form 4 was inadvertently transposed to item 12a of her DD Form 214. 2. The evidence confirms she enlisted in the Regular Army and entered AD on 26 October 2005. The date 2006 01 05 which is currently shown is incorrect. She is therefore entitled to a correction of item 12a to show 2005 10 26. 3. Her DD Form 214 shows in item 12b that she separated on 2008 05 01. The governing Army regulation states the total amount of net active service is computed by subtracting item 12a from item 12b. The difference is then entered in item 12c. As a result, item 12c should be corrected to read, 0002 06 06. 4. As a result of amending item 12a of her DD Form 214, she is also entitled to have item 18 reflect the correct period she was in a DEP status. Item 18 should be corrected to read, 20050826 – 20051025.
