THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show one bronze service star on his Afghanistan Campaign Medal. He also requests that an award certificate for the Distinguished Service Medal be added to his Official Military Personnel File (OMPF) on the interactive Personnel Electronic Records Management System (iPERMS).
2. The applicant states that his retirement was effective 1 July 2007 and campaign stars for service in Afghanistan were approved in April 2008. He contends that he served in Afghanistan from 17 April 2002 to 17 March 2003 and that the Afghanistan Campaign Medal is already reflected on his DD Form 214. He also states that award of the Distinguished Service Medal was presented to him at his retirement and that the medal is reflected on his DD Form 214 but the award certificate is not in iPERMS.
3. The applicant provides deployment orders; a copy of his DD Form 214; and an award certificate for the Distinguished Service Medal in support of his application.
CONSIDERATION OF EVIDENCE:
1. Having prior active service, the applicant entered active duty on 8 June 1977. He retired in the rank of colonel on 30 June 2007.
2. The applicant's DD Form 214 shows, among other awards, the Distinguished Service Medal and the Afghanistan Campaign Medal as authorized awards. Item 18 (Remarks) on his DD Form 214 shows, among other entries, that he served in Afghanistan from 17 April 2002 to 17 April 2003.
3. The applicant provided an award certificate, dated 18 June 2007, for the Distinguished Service Medal for exceptionally meritorious service for the period 30 June 1997 to 30 June 2007.
4. A review of the applicants performance section of his OMPF on the iPERMS did not reveal a copy of the award certificate for the Distinguished Service Medal.
5. Army Regulation 600-8-22 (Military Awards) states that individuals authorized the Afghanistan Campaign Medal must have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all land area of the country of Afghanistan and all air spaces above the land. The Afghanistan Campaign Medal period of eligibility is on or after 24 October 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. The regulation authorizes award of a bronze service star based on qualifying service to be worn on the appropriate campaign or service medal, including the Afghanistan Campaign Medal. A bronze service star is authorized to be worn on the Afghanistan Campaign Medal for the Liberation of Afghanistan (11 September 2001 through 30 November 2001), Consolidation I (1 December 2001 through 30 September 2006), and Consolidation II (1 October 2006 through a date to be determined) campaigns.
6. Army Regulation 600-8-104 (Military Personnel Information Management/ Records) provides policies, operating tasks, and steps governing the OMPF. Table 2-1 of this Army regulation shows that documents and certificates that award badges, service medals, tabs or non-Army awards for which no orders are published are filed in the performance section of the OMPF.
DISCUSSION AND CONCLUSIONS:
1. The applicant participated in one campaign during his assignment in Afghanistan which makes him eligible for award of the Afghanistan Campaign Medal with one bronze service star.
2. Since the governing regulation states that certificates that award service medals are filed in the performance section of the OMPF, and the applicant's award certificate for the Distinguished Service Medal is not filed in the performance section of his OMPF, it would be appropriate to add this award certificate to the performance section of his OMPF on the IPERMS.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. amending his DD Form 214 to add one bronze service star for wear on his already-awarded Afghanistan Campaign Medal; and
b. adding the award certificate for the Distinguished Service Medal to the performance section of his OMPF in the IPERMS.
